Comparison Of Dialysis Treatment Modalities For End-stage Diabetic Nephropathy

ObjectiveTo assess the effect of different dialysis methods in ESRD patients with diabetic nephropathy (DN) and to provide optimal therapy for those patients.MethodsObservation of 78 cases of end stage renal disease undergoing dialysis treatment, including 25 DN patients on PD,24 DN patients on HD and 29 cases of non-DN on PD. Compare the general information, laboratory indexs and complications among those groups from the initial time to the following-up 1 year.Results1 year after dialysis, compare end-stage DN patients on HD with patients on PD, there were significant differents (P<0.05) in systolic pressure,24-hour urine volume. Serum albumin levels were significantly increased in both two groups. HD group showed higher morbidity of fatal complications. In initial period, there were significantly differents (P<0.05) in the average body weight, diastolic blood pressure, serum creatinine, and serum albumin level between DN patients on PD and non-DN patients on PD. After 1 year, the body weight and diastolic blood pressure were significantly different (P< 0.05) between the two groups, but all the other indexes were similar (P>0.05). The incidence of peritoneal dialysis-related peritonitis was similar in both groups (P> 0.05).ConclusionsFor end-stage diabetic nephropathy patients, peritoneal dialysis prevents more advantages than hemodialysis, such as blood pressure stability, protection of residual renal function, low incidence of fatal complications and so on. Peritoneal dialysis is confirmed safe and effective, and should be the optimal method as renal replacement therapy for end-stage diabetic nephropathy patients.
