ObjectiveTo summarize the experience in the diagnosis and treatment of mucinous cystic neoplasms(MCN) of the pancreas.MethodsRetrospective analysis was made on the clinical data of 34 cases of pancreatic cystic neoplasms in the first affiliated hospital of China medical university from January,1999 to July,2009.ResultsThe clinical manifestations mainly included abdominal pain or discomfort, back pain; 12 patients had abdominal tenferness,8 patients had palpable abdominal mass. Ultrasonography and CT diagnosed correctly in 72% and 80% of all patients respectively, but they could not be used for tumor differentiation; the value of using serum tumor markers was unclear. The tumors were mostly located in the body and tail of the pancreas, with a mean diameter of 2~18 cm. All the 34 cases received surgical treatment. The procedures mainly included spleen-preserving distal pancreatectomy and rsection of pancreatic body and tail. The main operative complication was pancreatic fistula(24%); all the fistula was cured. Pathological examinations confirmed there were 24 patients with benign MCN of the pancreas,2 were borderline mucinous cystadenomas and 8 mucinous cystadenocarcinomas. Patients with invasive carcinoma were significantly older than those with noninvasive neoplasms (56 vs.45 years, P< 0.01). Findings associated with malignancy was diameter> or=60 mm (P< 0.001). After a mean follow-up of 28.3 months, patients with benign MCN of the pancreas or borderline mucinous cystadenomas were still alive without recurrence, the five year survival of patient with mucinous cystadenocarcinomas. ConclusionsMCN of the pancreas mainly occurred in women of perimenopausal age group, and there was no-specific clinical features. The tumors were mostly located in the body and tail of the pancreas.
