| Background:Rheumatoid arthritis is an autoimmune disease that mainly causes inflammation of the peripheral joints in multisystem. During the course of disease, patients suffered from severe joint pain which is characterized by symmetry, progressive and destructive articular damage, and finally appears joint deformities which result in disability in different degree(about 1/3 will develop to disabled). It is a clinically common and refractory disease in the world. So far there is no perfect therapy. According to the report from the joint foundation, the incidence rate of RA was 1%-3%. The surveys showed that the incidence rate of RA was 0.44% in Shenzhen, which was slightly higher than the domestic results, but obviously lower than white(2.0%),and was higher in women than in men, of the men and women than be 1:39. This disease sends various age groups crowd morbidity, but the targeted age group was from 20 to 44 years old young adults. Currently, western medicine used in rheumatoid arthritis has anti-inflammatory and analgesic effects significantly, however, as well as the side effect. No single non-steroidal anti-inflammatory drug (NSAID) was absolutely safe. TCM:The therapeutic mechanism of Chinese medicine decoction is to suppress immunity by using toxic Chinese medicine mainly, but it takes much time to decoct herbal medicine. Acupuncture therapy is a simple and safe way, but it also needs the patients take a long time and constant treatment. Therefore, a green treatment to save time with dual efficacy of drugs and meridian points is an urgent matter worth finding.Objectives:This research selected 50 patients accorded with the diagnostic criteria and Exclusion criteria of RA which were allocated to the observation group and the control group randomly. Patients in the observation group were given spinal moxibustion therapy and in the control group were given the cooperation of acupuncture(Jiaji points) and TCD radiation, to evaluate the acupuncture effects of spinal moxibustion therapy for RA. Using the cytokine IL-â… and IL-15, RF and Hb, ESR and CRP as observation index and observing the changes of parameters before and after treatment to discuss the relativity between spinal moxibustion therapy and improvement of the immune system on patients with RA, as well as to provide a theoretical basis for spinal moxibustion therapy for RA.Patients and methods:50 patients with rheumatoid arthritis were strictly selected, according to diagnostic criteria, inclusion criteria and exclusion criteria, and were randomly divided into 2 groups. Patients in the observation group were given spinal moxibustion therapy and in the control group were given the cooperation of acupuncture (Jiaji points) and TCD radiation. In the observation group they were asked to lie down on their stomach with naked back on the bed. After routine disinfection on the skin below the Spinal process from first thoracic vertebra to fifth lumbar vertebra, the doctor strip spread ginger juice upon the skin along the spine (about 2cm wide,2cm height), and scattered the spinal moxibustion powder (nano-grinding musk, gongdingxiang, chongweizi and weilingxian in the ratio of 1:15:15:15) linearly and evenly on it, then applied a mulberry paper about 18cm wide, on which covered with 5cm wide and 2cm height ginger shaped stepwise. Finally, put the spindle-shaped cones on top in sequence. And then lit the top, the middle and the button of the cones by pastille until off. After renewing the cones 3 times, removed the ginger and cleaned with a hot towel. The skin became reddish and blistered after 4-6h (Size or quantity varies with the individual).Then discharged it after 24h and waited it to scab. Once every four weeks makes up one course. The observation continuously lasted for 3 courses.In the control group:Patients were asked to lie down on their stomach with naked back on the bed. After routine disinfection on the skin below the Spinal process from first thoracic vertebra to fifth lumbar vertebra, the doctor administered the acupuncture therapy on jiaji points from the neck to fifth lumbar vertebra with uniform reinforcing-reducing method and reducing needling technique with lifting and twirling, and that were maintained for 30min with TCD radiation. Once every day, with an interval of one week at the forth week and go on for next session. The observation continuously lasted for 3 courses. The changes of parameters and the evaluation of clinical effect before and after treatment between both groups were statistically compared after the whole process.Results:There was a statistical significant difference between two groups in clinical symptoms, improvement of the physical sign, overall evaluation of pain from the patients and overall evaluation of the arthritis from doctor and patients before and after treatment (p<0.01). The changes of parameters in the observation group showed obviously more than in the control group. The difference had statistical significance between two groups (p<0.05). In terms of curative effect, there were 6 cases (24%) with ACR70, 14cases (56%) with ACR50,4cases (16%) with ACR20, and 1 invalid case (4%) in the observation group, hile in the control group, there were 3 cases (12%) with ACR70,9 cases (36%) with ACR50, llcases (44%) with ACR20, and 2 invalid case (4%). The difference had statistical significance between two groups (p<0.05). In comparison of the Hb, IL-â… , IL-15, RF, ESR and CRP between two groups before and after treatment, the parameters of IL-â… ,IL-15, RF, ESR and CRP in both groups declined after the treatment but the Hb increased. The difference had statistical significance between two groups (p<0.01). In addition to CRP, results of parameters before and after treatment showed significant difference (p<0.05)Conclusions:The treatment of spinal moxibustion and the cooperation of acupuncture (Jiaji points) and TCD radiation both have effect on patients with RA, and the spinal moxibustion therapy is more convenient and effective. Through the treatment of spinal moxibustion and the cooperation of acupuncture(Jiaji points) and TCD radiation, the symptom and physical sign, as well as the immune system on patients with RA were improved significantly. The level of IL-â… , IL-15, RF, ESR and CRP was declined while the level of Hb was increased. It demonstrated that the treatment of spinal moxibustion and the cooperation of acupuncture (Jiaji points) and TCD radiation both improved the immune system on patients with RA, and the spinal moxibustion therapy is more convenient and effective. That provided a theoretical basis for spinal moxibustion therapy for RA.
