Objective:To Analyze the prognostic factors of postoperative radiotherapy in initial treatment patients with malignant glioma.Including:the factors of patients:the age and KPS;the factors of tumor:the size, location and pathologic patterns;the factors of treatment:the boundaries and surrounding in surgical resection,the radiation total dose in the radiation dissection modality,the surrounding of CTV, combined chemotherapy or not,continuous treatment or not. In order to find the independent prognostic factors affecting tumor recurrence time and long-time survival of malignant glioma patients, then guiding clinical treatment better.Methods:This was a retrospective study.There were 131 initial treatment patients with postoperative malignant glioma were retrospectively joined this study. The clinical data of 131 patients with malignant glioma were proved pathologically. Analyzing the prognostic factors affecting tumor recurrence time and long-time survival of malignant glioma patients. Kaplan-Miere survival curve and Cox regression model were applied to evaluate the overall survive rate and the prognostic factors respectively.Results:The median time to relapse was 10.7 months, and the overall median survival was 11.7 months.The overall 1,3,5 year survival rate were 38.8%,17.6% and 9.8%respectively. The boundaries and surrounding in surgical resection,the radiation total dose,combined chemotherapy were the main prognostic factors affecting tumor recurrence time (P<0.05, partial regression coefficient was negative). The age,KPS, the boundaries and surrounding in surgical resection, continuous treatment were the main survival-related prognostic factors (P<0.05).Conclusions:(1)For the initial treatment patients with postoperative malignant glioma, the main prognostic factors affecting tumor recurrence time were the factors of treatment., including complate resection, total dose≥70Gy, combined chemotherapy,which can postpone the risk of tumor recurrence(2)For the initial treatment patients with postoperative malignant glioma,the age≤40,KPS≥70分, the boundaries and surrounding in surgical resection, continuous treatment were the main survival-related prognostic factors,which can. (3)The factors of tumor has no more relationgship with the prognostic.Compared with these,the factors from patients and treatment were more important.
