Objective To investigate the serum level of brain-derived neurotrophic factor(BDNF) and Interleukin-2, IL-6 among patients with Depression and Obsessive- compulsive Disorder,and analyzing their relevance to explore the possible role of BDNF and IL-2, IL-6 in Etiology and Therapy.Method1. Case selection and diagnostic criteria:From October 2008 to December 2009,We selected 40 cases of hospitalized patients during the mental health center in Qingdao, and divided them into obsessive-compulsive disorder group, 20 cases and depression group, 20 cases. Obsessive-compulsive disorder and depression are in line with DSM-IV and CCMD-3 diagnostic criteria. These cases were required to age for 18-60 years old, 10 cases each for men and women, secondary or higher education, normal level of audio-visual. Total score of HAMD-24 is greater than or equal 20 in depression group,the score of Y-BOCS is greater than or equal 16 in obsessive-compulsive disorder group. The normal control group included 20 health staff.2. Treatment:Depression group were given paroxetine 10mg per day in the first week, increased 30mg per day in the second week; obsessive-compulsive disorder group were given paroxetine 10mg per day in the first week, increased 40mg per day in the second week. The treatment had lasted 8 weeks. Two groups were giving psychological treatment at the same time.3. Collection of serum samples:Before ues no drugs, collecting 5ml forearm blood,and centrifugal, aggregation, collecting serum after the natural cryopreservation, for spare. Collect again after the treatment of 8 weeks.4. Determination:Operate in accordance with the kit manual by ELISA. Apply to the sample, measuring OD450 on a MULTISKAN MK3 microplate reader, and calculating concentrations of BDNF and IL-2, IL-6.5. Statistical Methods: Statistical software SPSS13.0 was used to analyze Data. A comparison between the two groups selected independent samples t-test; measurement data of self-control used paired t-test, correlation analysis among multiple variables used method Spearman. P <0.05 or P <0.01 indicated that the difference was statistically significant.Result1. The change and clinical significance of serum BDNF levels in depression and OCD patients, before and after treatment1.1 Before the treatmentBDNF serum level of depression group is 20.17±10.20ng/ml, 19.85±11.67ng/ml in obsessive-compulsive disorder group and 39.02±15.46ng/ml in normal control group. The difference between patients group and control group was statistically significant (P<0.05). The difference in serum BDNF levels between Depression group and obsessive-compulsive disorder group is no significant (P>0.05).1.2 After the treatmentAfter 8 weeks of standardized drug treatment, with psychological treatment, serum BDNF level of depression group increased significantly (38.21±14.53ng/ml), is no significant difference with the normal control group (P>0.05); serum BDNF levels of obsessive-compulsive disorder group increased significantly (27.79±10.65ng/ml), is still higher than the normal control group, and the difference is statistically significant (P<0.05).1.3 The relationship between Serum BDNF levels, disease course and symptomsAfter 8 weeks of antidepressant treatment, there are 12 patients effect significantly in depression group, effective 6 and ineffective 2. After 8 weeks of anti-forced treatment, there are 8 patients effect significantly in OCD group, effective 7 and ineffective 5. Serum BDNF levels of obsessive-compulsive disorder has nothing to do with severity of obsessive-compulsive symptom, serum BDNF levels of depression was negatively correlated with scores of HAMD(r =-0.942,P =0.005). Serum BDNF levels of them are both related to the course of diseases.2. The serum levels of IL-2, IL-6 and the clinical significance in Depression and obsessive-compulsive disorder patients, before and after treatment.2.1 Before the treatmentThe serum level of IL-2 in Depression group is 4.52±2.36μg/L, the serum level of IL-6 is 38.45±12.71ng/L. They are all higher than normal control group (IL-2, 2.13±0.98μg/L; IL-6, 27.34±13.62ng/L), and the difference is statistically significant (P <0.01). The serum levels of IL-2 in OCD group is 4.27±1.85μg/L, and it is higher than normal control group (P <0.01). The serum level of IL-6 is 26.91±11.53ng/L, there is no significant difference with the normal control group (P>0.05). The serum level of IL-6 in Depression group is significantly higher than the OCD group (P <0.01).2.2 After the treatmentAfter 8 weeks of treatment, the serum IL-2, 6 levels of depression group are lowered, the serum level of IL-2 is 2.37±1.69μg/L, and the serum level of IL-6 is 28.34±11.36ng/L, there is no significant difference with the normal control group (P>0.05). The serum levels of IL-2, IL-6 in obsessive-compulsive disorder group were 3.15±1.47μg/L and 27.04±12.85ng/L. The serum level of IL-2 is higher than the normal control, and the difference is statistically significant (P<0.05). Serum IL-6 levels showed no significant difference before and after treatment (P>0.05).2.3 The relationship between levels of serum IL-2, IL-6 and clinical symptomsThe serum levels of IL-2, IL-6 in Depression is positively related to scores of HAMD (P<0.05). The level of serum IL-2 is related to obsession(r =0.835,P =0.005). The level of serum IL-6 is related to the severity of symptoms and compulsory.Conclusion:1. Patients with Depression and Obsessive-compulsive Disorder have lower serum BDNF levels than normal population. After standardized therapy with paroxetine and psychological treatment, the level of serum BDNF in depression patients is near to normal, while it is lower than the normal level in OCD patients.2. Serum BDNF level in Depression is negatively correlated with scores of HAMD. Serum BDNF level in Depression and OCD are both negatively correlated with the course of diseases.3. Serum levels of IL-2, IL-6 in Depression are higher than normal. After the treatment, serum levels of IL-2 and IL-6 reduced to the normal. They are both positively related to scores of HAMD.4. The serum level of IL-2 in OCD is higher than the normal. After the treatment, it is decreased, but is still higher than the normal. The level of serum IL-2 is related to obsession, and the level of serum IL-6 is related to the score of Y-BOCS and compulsory.
