The Pulse Characteristic And Clinical Significance Of 542 HIV/AIDS Cases

[Background]AIDS is a viral infection disease, because of the HIV virus invades the body. The virus caused the cellular immune function decline and complex infection of multi-system, multi-organ, multi-pathogens. The clinical-manifestations of AIDS is very complexity and diversity, long course, high mortality. In TCM, AIDS is similar to "phytophthora disease", but the AIDS also is special "phytophthora disease", caused by incubative pathogenic factors-HIV virus. The characteristics of HIV virus are stronger and wider infection than the other plague viruses, many HIV infectors have similar clinical syndromes, and the most important characteristics of HIV virus is concealed, it means the infectors will not appeared syndromes and physical signs, because the HIV virus concealed in infectors body (Ying-xue stage in TCM). When immunity of infectors became weaker than before, the HIV virus carried out copy in high-speed, the infectors will appear clinical syndromes and sick in prognosis.The pulse diagnosis is quite importance diagnosis way in TCM. The pulse diagnosis has been more complete and systematic experienced about 2,000 years. The pulse diagnosis is a way of high sensitivity, simple to operate, no pain and no injury, made it easy to be accept. However, this way of diagnosis still has it's defection:just like the information of pulse can only touched by doctors, not only lake of objectivity and hard to master, but also hindered the development of pulse theory caused no record be saved. So, we have to using modern skills and instruments to recording information of pulse, studying theory of pulse, it's the good way to developing study of pulse.So, we selected the ZM-BOX-Ⅰtype intelligence electro-pulsograph, which analyze and inference the data of the 542 cases HIV/AIDS patients'pulse location, the force, the tendency, the shape, the rate, the rhythm. We have been combined with literature about pulse of HIV/AIDS people to summarize common pulses, relationship between CD4+T lymphocyte count and pulse of HIV/AIDS patients, to supply objectively evidence for AIDS in TCM syndromes and types.[Objective]Through the comparative study of 542 HIV/AIDS patients about their pulse tracings map (include principal parameter, pulses location, pulses force, pulses tendency, pulses shape, pulse rate, rhythm and so on), and the approach about the relationship between CD4+T lymphocyte count of HIV/AIDS patients and the characteristics of their pulse tracings maps, for illuminating the characteristics of common pulse tracings of HIV/AIDS patients and the relationships between the pulse tracings and the progression of pathogenetic condition which reflected by CD4+T lymphocyte count, then, providing objective basis about the TCM dialectical cure and the judgement of effect of AIDS.[Subjects]Subjects 542 HIV/AIDS patients all come from Beijing-Ditan hospital,Youan hospital and country area of lincang city in Yunnan province, the patients in Beijing city, their route of infection most is sex of homosexural, and the patients in Yunnan province, their route of infection is take drug by common syringe and providing blood for money. There are 178 cases AIDS patients and 364 cases HIV/AIDS infectors. So far, the 178 cases AIDS patients taked the therapy is HAART combined with Traditional Chinese Medicine (TCM for short), the other 364 cases HIV/AIDS infectors taked the therapy is TCM.[Methods]1 We selected the 542 cases HIV/AIDS patients and collect pulse tracings of the guan pulse of the patients through the ZM-BOX-Ⅰtype intelligence electro-pulsograph, which analyze and inference the data of the location,the force,the tendency,the shape,the rate,the rhythm and so on to constructing the tracings map and various kinds of parameter information, then we deposit the collected data to the computer.2 Depend on CD4+T lymphocyte count≤200/mm3,and>200/mm3,the HIV/AIDS patients are divided to two groups (called①group,②group for short), then,their pulse tracings data are analysised respectively, we obtain the changed characteristics and the difference of the pulse tracings when under different immune function. Then we try to explain the mechanism through the pulse diagnosis. Through the comparative study of the AIDS patients group (①group) and the HIV/AIDS infectors group (②group) about the outcome data, we uncover the characteristics of the pulse tracings of the two groups and the patho-characteristics of the infected patients.3 The statistical analysis is adopted t tests, chi square test and correlation analysis of SPSS11.0 software.[Results]①The simple purse of the 542 cases, the detection rate of excess-pulse (79.7%) is significantly higher than deficiency-pulse; The most detection rate of excess-pulse (64.6%) is wiry-pulse, the rate of deep-pulse (44.2%) is less than wiry-pulse, the detection rates of the slippery-pulse, rapid-pulse and floating-pulse are less than deep-pulse. The complexity pulse of the 542 cases, the detection rate of the deeply-wiry pulse is the most (36.8%), the rate of the deeply-slippery pulse, the rate of the deeply-slow pulse, the rate of the wiry-rapid pulse and the rate of slippery-rapid pulse are less than deeply-wiry pulse.②In the group of CD4+T≤200mm3,the detection rate of the excess-pulse(82.8%) is significantly higher than deficiency-pulse (P<0.01).The most detection rate of excess-pulse is the rate of wiry-pulse (48.3%), the rate of deep-pulse(37.9%) is less than wiry-pulse, the rate of slippery-pulse and the rate of the rapid pulse are the same (27.6%), the rate of deficiency-pulse (8.6%)is most less than excess-pulse; In the group of CD4+T>200mm3, the detection rate of the excess-pulse(81.3%) is still significantly higher than deficiency-pulse (P<0.01). The most detection rate of excess-pulse is the rate of wiry-pulse (48.4%), the rate of deep-pulse(34.1%) is less than wiry-pulse, the rate of the slippery-pulse is 26.9%, the rate of the rapid-pulse, the rate of slow pulse and the rate of floating-pulse are less than the slippery-pulse, the rate of deficiency-pulse is 8.5%.[Conclusions]HIV/AIDS infectors often have the excessive syndrome-type, and the most is the young people in HIV/AIDS infectors, so their pulses are powerful, the detection rate of the excess-pulse, wiry-pulse and the slippery-pulse are the most even in disease progression; AIDS patients are more weaker than HIV/AIDS infectors, so we must understand the real etiology of their excess-pulse's. In TCM, because the patients, their Qi, Blood, Yin, Yang all are deficiently, it means the immunity of the patients is extremely low, in this situation, the body generate pathological products like blood stasis, fire, phlegm and fluid, we called this situation "excess syndrome caused by deficiency". So, while these patients detected excess-pulse, it doesn't mean their immunity strong.
