Objective Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common inherited renal diseases. The incidence is 1:400~1:1000. It causes 1/10 of all reanl failure. Polycystic kidney disease progresses slowly but persistently, and eventual end-stage renal failure is common. The aim of the study is to determine the expression of osteopontin (OPN) in ADPKD renal tissues, and to research the function of OPN in the pathogenesy and progress of ADPKD.Methods Cases were from hospital patients in urinary surgery of Provincial Hospital Affiliated to Shandong University from 2007 to 2009.67 samples of patients removed in unroof depressing operation were chosen as cases of experiment group,38 cases were male,29 cases were female,the age were from 29~65, and the mean age was 49.24 cases of normal renal tissues were chosen as control group.(1) The expression of OPN was assayed by means of immunohistochemical technique of SABC method. According to the different staining levels, expressions were scored positive, weakly positive and negative. Statistical method was performed to analyze the difference of OPN expression between normal renal tissues and polycystic kidney tissues.(2) All the glomerular filtration rate of operative side kidney were recorded and divided into 2 GFR groups:A(50ml/min>GFR≥25ml/min), B(25ml/min>GFR> 1Oml/min). Statistical method was performed to analyze the dependablity between GFR and the levels of OPN in podocyte.(3) All the kidney volume(KV) of ADPKD patients were recorded and divided into 2 KV groups:A(kidney volume≤750cm3), B(kidney volume>750cm3). Statistical method was performed to analyze the dependablity between KV and the levels of OPN in renal tissues.(3) All the blood pressure of ADPKD patients were recorded and divided into 2 BP groups:A(systolic pressure 110~159 mmHg or diastolic pressure75~99 mmHg), B(systolic pressure≥160 mmHg or diastolic pressure≥100 mmHg). Statistical method was performed to analyze the dependablity between BP and the levels of OPN in renal tissues.Results (1) The expression of OPN was seen in distal tubular epithelial cells, and was usually most prominent along the luminal side of the cell. And in ADPKD kidney tissues, OPN was localized most intensely within markedly cystic epithelium, while OPN expression by non-dilated, non-cystic tubules was relatively limited.(2) In GFR groups A and B, the total positive expression rates of weakly positive expression were 62.2% and 26.7% respectively, with positive expression were 37.8% and 73.3% respectively. The levels of OPN significantly increased with the decrease of GFR(P<0.05).(3) In KV groups A and B,the rates of weakly positive expression were 58.8% and 33.3% respectively, with positive expression were 41.2% and 66.7% respectively. The levels of OPN significantly increased with the increase of KV (P<0.05). (4) In BP groups A and B,the rates of weakly positive expression were 62.9% and 28.1% respectively, with positive expression were 37.1% and 71.9% respectively. The levels of OPN significantly increased with the increase of BP (P<0.05).Conclusion (1) The expression of OPN in ADPKD renal tissues was localized most intensely within markedly cystic epithelial cells and distal tubular epithelial cells, while OPN expression by non-dilated, non-cystic tubules was relatively limited. OPN was weakly positive in normal kidney tissues by immunohistochemistry staining.(2) The levels of OPN in ADPKD renal tissues show a positive correlation with kidney volume and blood pressure of ADPKD patients. The expression of OPN in ADPKD renal tissues show a negative relationship with GFR of operative side kidney. Accordingly, OPN might play an important role in the pathogenesy of ADPKD.
