Objective:To explore the surgical treatment of papillary thyroid carcinoma(PTC) of the effect in the initial and resurgery patients.Methods:Retrospective investigation was carried out in the Department of Head and Neck Surgery from January 1995 to December 2007, diagnosed and done radical mastectomy of 118 cases of thyroid papillary carcinoma.Inclued 58 cases of initial diagongsed,and 60 cases who needed reteat with surgery, whose clinical data were colleted. The situation of cervical lymph node metastasis,complication rate,the recurrence rate in 5-year and mortality in both groups were Compared,and to do survival analysis.We accessed the imformations of the patients through medical records, telephone follow-up, out-patient review.Results:118 patients without surgery related death.The cervical lymph node positive rate of the first group was 51.7% and the second group was 65.0% in the cervical lymph node dissection. The complications after surgery surgical was 6.9% (4/58)in the first group and20.0% (12/60) in the second, There were significant differences between the two (P<0.05). Follow-up process, died 11 cases,2 patients were lost,4 patients died of local recurrence, neck recurrence was 2 cases and lease died of distant metastasis. The 5-year recurrence of the first operation was 5.2% (3/58), and 18.3%(11/60)in the second, the difference was significant statistically (P<0.05). Postoperative follow-up, the first group's 5-year survival rate was 96.6% (56/58)and 10-year survival rate was 94.8% (55/58). The second group's 5-year survival rate was 90.0% (54/60) and 10-year survival rate was 83.3% (50/60). There were not significantly statistically significant in 5-year survival between the two groups (P> 0.05).But 10-year survival rates are different (P<0.05).Conclusion:As to the papillary thyroid cancer patients who needed surgery again, postoperative residual tumor recurrence rate was high. therefore,in order to improve the patient's free survival, thyroid cancer surgery need to select a standardized way to improve the effectiveness of surgical treatment in the first time.It suggested that early detection and standardized surgical treatment can achieve satisfactory effect in the initial diagnosis.
