Evaluating Coronary Artery Stenosis Using 256-slice Speed CT

Objective:To assess the diagnostic accuracy and the follow-up value of Philips Brilliance ICT 256 layers in the coronary artery stenosisMethods:72 patients suspected coronary heart disease in our hospital are carried out coronary artery imaging by Philips 256 layer high speed CT. The results of all the suspected cases and the cases after stent were analyzed by two experienced doctors together, compared with coronary angiography. Then we evaluated the sensitivity, specificity, positive predictive value and negative predictive value of speed CT in the diagnosis of coronary artery stenosis (grade>50%).Result:There were 14 cases whose coronary artery were normal,27 cases whose coronay stenosis rate was less than 50%,21 case were more than 50%and less than 70%,and 10 cases were more than 70%among the patients suspected coronary heart disease. The sensitivity is 98.27%, and the specificity is 93.75%. The positive predictive value is 100%, while negative predictive value is 93.75%. The cases include 20 patients who have taken PCI six months ago, and 39 stents were implanted. Coronary angiography were underwent in the patients.The results of CAG showed that in-stent restenosis rate of 5 cases were more than 50%, while 6 cases showed in speed CT. In-stent restenosis rate of 3 cases were less than 50%, and 4 cases in CT, There were 31 cases with no restenosis in CAG, while 29 cases in speed CT. The sensitivity is 100%, and the specificity is 93.94%. The positive predictive value is 80%, and negative predictive value is 100%.Conclusion:The Speed CT have high accuracy in diagnosis of coronary artery stenosis, and it can be use as a noninvasive technique for evaluation of coronary artery stenosis and stent follow-up after six months.
