Objective:Histiocytic necrotizing lymphadenitis(HNL) is a highly misdiagnosed and rare disease which is paid much more attention to by the clinicians in recent years. This study will discuss the differential diagnosis of HNL through its clinical manifestations, laboratory tests and pathological features.Method:Analyze 20 HNL patients retrospectively.Results:17 patients (85%) were misdiagnosed in other hospitals. The main clinical features include fever (100%), lymphadenopathy (100%), with tenderness (55%), increased erythrocyte sedimentation rate (75%) and decreased leukopenia (55%). Histopathology shows the necrosis of cortex or paracortex of lymph node, with histiocytes and lymphocyte proliferation, nuclear debris and without infiltration of neutrophils.Conclusion:Histiocytic necrotizing lymphadenitis has tipical pathological changes, but we should pay attention to misdiagnosis.
