Objective To explore the clinical outcome of total hip surface replacement for the treatment of osteonecrosis of the femoral head in young and middle-aged.Methods From Mar.2004 to Sep.2009.16 consecutive patients(18 hips) with osteonecrosis of the femoral head underwent Total Hip resurfacing replacement.14 cases(15 hips)were diagnosed Ficat III, and 2 cases(3 hips) were diagnosed FicatⅣ. The average age at the time of surgery was 47 years(41~55 years).6 patients(37.5 %) were male and 10(62.5%) were female. Clinical and radi- ographic results were observed.Results All patients were followed up for a mean of 37 months (6~72 months), During the follow-up period, no complications such as dislocation of the hip joints, deep venous embolism, infection or femoral neck fracture occurred. The mean postoperative Harris hip score was 91.1 to 44.5 preoperatively. The outcome of 17 hips were excellent,1 good, and none failed. The radiographic analysis showed all prostheses were fixed. The average femoral neck-shaft angle increased from preoperative 134.1°to postoperative 141.2°.The average preoperative anteversion of femoral neck was 34.5°The average abduction angle was 42°. The limb discrepancy decreased from preoperative 1.3 cm to postoperative 0.5 cm. Besides the pain is nearly completely relief, the range of motion is remarkable improved.Conclusion Total hip surface replacement is one of the best choice for the treatment of osteonecrosis of the femoral head in young and middle-aged, and gets satisfactory result.
