Sneddon's syndrome (SS) is a rare neurocutaneous syndrome, which is characterized by the combination of ischemic cerebrovascular diseases and a wide range of skin livedo reticularis, accompanied by heart, kidney and other organ damages. It is a systemic disease. We reported a case of young man, who was admitted for acute right hemiparesis, with a history of skin livedo reticularis and a history of hypertension, antiphospholipid antibody negative. He was thrombolyzed by intravenous recombinant tissue plasminogen activator (rt-PA) with the assessment of MRI diffusion and perfusion imaging. This is the first report of intravenous thrombolytic therapy in SS.
