Objective:To evaluate the clinical effect of fiber post on flared roots after the rehabilitation by composite resin.Methods:Choice 11 patients who require repair weak residual roots in the Comprehensive department of the Affiliated Stomatologic Hospital of Zhejiang University from August 2009 to March 2010. After detailed oral examination laboratory examinations and other preparation treatment, including remove the original crown, metal post, complete root canal therapy and periodontal treatment. With no discomfort after one week, remove caries and reserve healthy tissues, remove the gutta-percha and retain about 4mm nearly the apical part with G drilling in the root canal, according to the length measured in the process of root canal treatment, then choose the right size of glass fiber post according to the diameter of the roots, prepare the root canal by the same size drill, and insert the glass fiber post into root canal, to make sure the space between the glass fiber post and dentin. Clean the root canal wall and glass fiber post, bond the glass fiber post with Rely X Unicem, remove the redundant agents, light-cured 40 seconds and fix the post in the appropriate location. Coating the root canal wall and dentin with Clearfil and light-cured 20 seconds then fill the gap between the root canal wall and the glass fiber post with composite resin, moulding the shape of the core, light-cured and prepare according to the requirements of tooth preparation, then gingival retraction, impression, make temporary crown. At last, try on the crown, occlusal adjustment, polish and bond. Inform patients the related notes. Later observe the clinical effects.Results:①The results showed that the restorations were appropriate with the parapost glass fiber post, composite resins and resin cement. All the restoration were margin sealed, good retention, good appearance and satisfactory. In the later visit, all roots were firmly intact, no root fracture, no periodontal problem, no gum stain and allergy; also all restoration were good retention, no marginal seal, no loosening off, no porcelain collapse, the clinical effect is good.②Restore the partially weakened roots and largely weakened roots with flowable resin, composite resin differently both receive good clinical results in the short time observation.③The application of Rely X Unicem Resin Cement and Clearfil bonder can ensure sufficient bonding strength in the interface between dentin—glass fiber post and the dentin—composite resins, make sure the heterogeneous material to be a mechanical complex after rehabilitation and enhance the fracture resistance of flared root canal and benefit for the preservation of flared root canal④In the rehabilitation of flared root canal, bond the glass fiber post firstly and then reconstruct with composite resin is feasible.⑤The fracture resistance of flared root after rebuilding is sufficient for chewing in the short term, but long-term effects should be observed.Conclusion:The application of composite resin and glass fiber post elastic modulus similar to dentin in flared root canal reconstruction and form the post-core for crown restoration have an ideal clinical effects in the short term observation.
