| ObjectiveTo study the clinical features of Hashimoto's encephalopathy in order to improve the diagnosis and treatment of this disease.MethodsThe Department of Neurology in our hospital from 2000 to 2009 were treated 16 cases of Hashimoto's encephalopathy in patients with retrospective analysis, and its clinical features, laboratory test results, diagnostic methods and treatment effect of the characteristics of such conduct.Results 16 patients,5 patients had thyroid disease, of which 1 patients had a family history of autoimmune thyroid disease. The clinical manifestations of cognitive dysfunction in 9 cases, the spirit of abnormal behavior in 10 cases,8 cases of extrapyramidal symptoms, seizures in 8 cases,9 cases of disturbance of consciousness, stroke-like episodes in 10 cases. By t-test or chi-square test reached the level of thyroid function for the impact of no difference in clinical symptoms.8 cases of lumbar puncture to examine cerebrospinal fluid in 4 cases of mild increase, other no abnormality.12 patients received EEG,9 cases showed moderate to severe abnormalities.13 patients received MRI imaging examination only 1 case prompted an unusual change. Serum Anti-TPO and Anti-TG-positive rate was 100%, the level of thyroid antibodies for the diversity of clinical symptoms through the use of linear regression analysis, no correlation.16 cases of patients had received glucocorticoid treatment for 1 case of combined application of immune globulin, have improved the symptoms improved. ConclusionHashimoto's encephalopathy is a complex and diverse clinical manifestations, objective examination of the disease specificity is not strong, there may be missed or misdiagnosed, it should raise awareness of the disease. When confronted with difficult to explain unexplained encephalopathy, should be noted that detection of serum anti-thyroid antibodies were significantly higher when the concentration should be a high degree of suspicion of the disease in a timely manner in order to glucocorticoid treatment, in order to achieve the best therapeutic effect. |
