ObjectiveBuild up the minimal change disease (MCD) model with adriamycin-induced nephropathic rats and interfere by mycophenolate mofetil (MMF).Examine the expression of nephrin in renal tissue.Inquire into the mechanism of podocyte hurting to produce proteinuria and the therapeutic effect of MMF.Methods36 male Wistar rats were divided at random into 3 groups:group A (adriamycin nephropathy group), group B (treatment group with MMF) and group C (normal control group), each one of even 12.At the beginning of the experiment (0 day), group A and group B were given a single tail intravenous injection of adriamycin (6.5 mg/kg) to establish the model of nephropathy,according to the Bertain's method,group C were given physiological saline in equal quantity.From the next day (the 1st day of experiment),group B were infused stomach with MMF (20 mg/kg·d, everyday once, total 28 days),group A and group C were infused 0.4 ml physiological saline everyday at the same time. At the 0,7,14,21,28th day of the experiment,all the rats were placed in metabolism cage to collect their urine for 24 hours and to measure 24h urinary protein count. At the 14,21,28th day of the experiment,four rats of each group were executed to get renal cortex.Observe the renal tissue pathological changes by light microscopy and electronic microscopy and examine the expression levels of nephrin.Adopt MetaMorph offline image-analysis system to semiquantitative analysis by immunohistochemistry and compute the relative content of nephrin.Data applied SPSS13.0 statistics software processing.P<0.05 had a statistic meaning.ResultsAt the 14th day of experiment,group A and group B nephrosis model created success.①Body weight:There was no significant difference among the three groups when the experiment started.From the 7th day to the end,group C weight gradually grew with feed,group A and group B weight descended gradually compared with group C (P<0.001), there was no significant difference between group A and group B.②24h urinary protein count:There was no significant difference among the three groups when the experiment started.From the 14th day to the end,group A and group B urinary protein increased obviously compared with group C (P<0.05).Group A urine protein excretion was gradually elevated and reached its peak at the 28th day.Group B urine protein was always lower than group A (P<0.05),maximized at the 21th day and decreased afterward,compared with the 21th day to obviously descend on the 28th day.③Pathologic change:Light microscopy showed that kidney glomeruli and tubules of group A and group B in any period was compared with group C without the obvious difference as usual.Electronic microscopy showed that glomerular basement membrane (GBM) in group C was complete and footprotrusion was clear,extensive fusion of footprotrusion in group A and partial fusion of footprotrusion in group B.④Relative content of nephrin:Immunohistochemistry pointed out the nephrin of group A and group B reduced obviously compared with group C (P<0.05) from the 14th day, Group B nephrin was more obviously high than group A (P<0.05).⑤Nephrin and proteinuria excretion related analysis:It was a negative correlation between nephrin and 24h urinary protein count.Along with urine protein recovered,the expression of nephrin as well resumed gradually.ConclusionWhen podocyte hurt, the expression of nephrin decrease.Along with condition recovered, the expression of nephrin as well resume.Nephrin expression and urinary protein count present negative correlation.MMF can relieve podocyte hurt and increae nephrin expression and then maintain integrity of podocyte's structure and function,attain to reduce proteinuria and defer kidney damage.
