The Value Of Practical Analyzing Regional Wall Motion Abnormality Of Left Ventricle By Echocardiography In The Non-ST-segment Elevation Myocardial Infarction

ObjectiveBy discussing the diagnosing value of two-dimensional echocardiography (2DE), approximately judging the severity of coronary artery and speculating the infarcted relative coronary artery in the acute non-ST-section elevation myocardial infarction (NSTEMI) through the results of 2DE,we evaluate the value of practical analyzing the regional wall motion abnormality (RWMA) of left ventricle by 2DE in NSTEMI.Method143 patients were diagnosed as NSTEMI. All of them first accepted 2DE and then catheter coronary angiography (CAG).Taking the results of CAG as the gold standard,and then evaluate the diagnosing value of UCG.Among 140 CAG positive patients(every stenosis degree>50% of coronary artery through CAG is CAG positive) according whether there is RWMA of left ventricle by 2DE,divided into 2DE positive group(93 cases) and 2DE negative group(47 cases).Then analysis the characteristics of lesion and CABG ratio of these two groups.24 cases were excluded in 2DE positive group,for there is no way to find out the infarcted relative coronary artery through CAG.69 patients were selected to evaluate the accuracy of 2DE in diagnosing NSTEMI compared with the results of CAG.ResultsThe sensitivity,specifity,positive predictive value,negative predictive value and accuracy of 2DE in diagnosing NSTEMI by the detection of RWMA is 66.4%,66.7%,98.9%,4.1%,66.4%;The results of CAG, single vessel disease, double vessel disease, triple vessel disease, mean stenosis degree of IRCA,IRCA complete occlusion and incomplete occlusion, CABG treatment, are all significant differences between the 2DE negative group and the 2DE positive group(P<0.05), collateral circulation formatted is no significant difference between them(P> 0.05); In the 2DE positive group,the accuracy of judging IRCA is:LAD (33/33) 100%, RCA (13/15) 86.7%, LCX (17/21) 80.9%, total accuracy (63/69) 91.3%.ConclusionsThough the accuracy of 2DE in diagnosing NSTEMI is not very high,we can estimate the stenosis degree of coronary artery by the detection of RWMA;And we can approximately judge the IRCA by the detection of RWMA in the 2DE positive group.
