The Clinical Study On Application Of Three-channel Self-sealing Sclerotomy In 20G Vitrectomy

Objective:To study the efficacy of three-channel self-sealing sclero-tomy in 20G vitrectomy.Methods:31 cases(31eyes) were underwent 20G in pars plana vitrec-tomy in our hospital from March of 2008 to June of 2009.For all the cases,the conjunctival flap that the shape of "inverse L" were made based on lmm posterior to the limbus.three-channel self-sealing sclerotomy were performed.At the end of the procedure, the cutter and light source were extracted firstly,attention must be focused on wound stability (sutured with a absorption 10-0 if persistent leakage),then extracted the infusion probe and obversed wound stability(gave the same procedure as before if leakage was found).The conjunctival incision closed by electrocoagulation in all cases.The follow-up was about 4-19 months,obversed the visual acuity of pre-and post-operation,IOP,and the wound situation,and the complications during intro-and poes-operation.Results:All the cases were procedured successfully.To the 93 sclero-tomies,85 did not require suturing while 8 instrument sclerotomies were closed by sutured at the end of the procedure, all of 31 infusion sclerotomies were closed completely after operation,Among the 8 sutured instrument wounds, two instrument wounds required to suture in 1 case while 6 cases were sutured only one wound. On the first day of the post-operation,conjunctival incision closed up well,there was few or none hemorrhage and no leakage in the subconjunctive, edema and congestion of the bulbar conjunctive was not obviously.The sclerotomy checked by the UBM after 2 weeks of the operation wre linear closed.Visual acuity was improved for all the cases.There were no complications like dialysis of ora serrata,peripheral retinal breaks,retinal detachment and vitreous hemor-rhage.Conclusion:three-channel self-sealing sclerotomy can be applied in 20G vitrectomy. Incisions without suture closure will be able to achieve on their own,and it's simple to perform and reduce the intraoperative time, lower complications related to the suture and recovery earlier, no additional instrument equipments and so on.It can be applied instead of 23G and 25G at the present stage.
