Retrospective Study On Clinical Treatment Effect Of Adult Mandibular Condylar Fractures

Objective:To compare the clinical effect of surgical treatment with conservative treatment in different classification of condylar fractures after treatment, so as to explore the differences between two treatment methods of condylar fractures and summarize their advantages and disadvantages of two treatment methods.Methods:Retrospective analysis of 38 cases accepting treatment of condylars fractures in Stomatology ward of First Affiliated Hospital of Dalian Medical University from 2006 to 2010.All patients were treated with surgical treatment or conservative treatment, such as Craniofacial used bandages, Intermaxillary traction, ORIF, titanium reset tension screw and post-oral functional exercise. Inofrmation needed to obtain was collected by consulting the medical records, telephone follow-up, outpatient appointment.38 cases of condyles fractures were classified respectively according to the position of the fracture lines(median, high, low) and displacement of condylar (classâ… , classâ…¡, classâ…¢, classâ…£), compared clinical treatment effect of each classification of surgery and conservative treatment through maximal mouth opening, joint pain, face type and limited movement of jaws, and statistics analysis each single factor.Results:The clinical effect of median, low fractures and classâ…¢fractures in the face type restoration and joint movement were statistically significant, surgical treatment effect is better than conservative treatment. The other group data statistics were no difference, but due to the influence factors on the summary, so it is difficult to obtain conclusion. Eccept class II and class III fractures and low fractures of surgical treatment, the average mouth opening of other groups were cured by the standard. Four patients were open bite, the others were returned to normal occlusal relationship.Conclusion:Two treatment methods of mandible condylar fractures each have their advantages and disadvantages,class I fractures and high fractures by conservative treatment can reach satisfactory effect.Surgical treatment of Median, low fractures and class III fractures were obviously superior to conservative treatment.Due to different direction and angle of displacement, classâ…¡and classâ…£fracture should be treated according to complex condition. Should be the resumption of temporomandibular joint function and shape, and face type,jaw function recovery, reduce complications, according to different conditions and multi-factor analysis option is correct choice of methods. Emphasis on treatment of post-oral functional exercise.
