| Objective:By using heparin gelatin sponge stickers on treatment traumatic perforation of tympanic membrane, to explore the mechanism that the heparin in the treatment of traumatic perforation of tympanic membrane and to access the efficacy of heparin in the treatment of acute perforations.Methods:62 cases (65 ears) of traumatic perforation of tympanic membrane were randomly divided into two groups:The test group had 37 patients (40 ears) and the control group had 25 cases (25 ears). According to the size of tympanic membrane perforation, the cases were divided into three subgroups, small(width<2.5mm), Middle (width 2.5mm-3.5mm, big(width> 3.5mm), the test group was supplied the treatment of heparin gelatin sponge stickers; The control group was used the traditional drying method. Both groups taken orally roxithromycin capsules one week and 0.15g each, twice a day. All the subjects were followed up for 4 weeks, observed and recorded the rate and healing period of traumatic tympanic membrane.Results:①During 4-week follow-up,39 ears were cured in the test group, the rate is 97.5%.14 ears were cured in the control group, the rate is 56%. The healing rate of test group was significantly higher than that of the control group(x2=17.602,P<0.001. The average of healing period is 10.59 days in the test group, and 23 days in the control group. There were significant differences in healing period between two groups. (χ2= 28.768, P<0.001).②Through the pure tone audiogram in 0.5kHz,1.0 kHz,2.0 kHz, the average of hearing threshold of air conduction increased in 15-25dB of 39 cured tympanic membrane in the test group, and above 15dB of 14 cured tympanic membrane in the control group.③All the morphology of cured tympanic membrane in the test group was complete, which was similar with the morphology of normal tympanic membrane.Conclusions:The heparin could effectively enhance closure of the traumatic perforation of tympanic membrane and can shorten the healing period of traumatic perforation of tympanic membrane, and the smaller of perforation, the faster of healing period. Heparin gelatin sponge stickers on treating traumatic perforation of tympanic membrane can be used as an economical, convenient, safe and effective method in clinic. |
