Research background:Chronic obstructive pulmonary disease is a disease that the etiological factor and pathogenesis are uncertain entirely, and it is a preventable, treatable disease currently. Smoking is one of the amongst major risk factors for the development of COPD,such as Genetic susceptibility, infection, occupational environment, air pollution and socio-economic status are also risk factors.At present,more studies have shown that COPD can lead to systemic inflammatory response,thus to participate in variety clinical manifestations,and smoking is the important main. Generally early smoking cessation intervention has become a recognized prevention.The qualitative diagnosis of COPD mainly depends on forced expiratory volume in one second/force vital capacity,so as the evaluation of lung function become the most important indicators of intervention effects,lung function testing can improve smoking cessation rates.Currently,early smoking cessation becomes the only effective tool on treatment COPD patients,especially early asymptomatic patients.It can also improve lung function and slow down the rate of decline in patients,but different patients have different improvement level. With the deepening of molecular biology, it has been educidated that such a complex gene disease as COPD exist the phenomenon of microsatellite DNA, single nucleotide polymorphism, etc. These genetic polymorphisms cause changes in the structure of protein, altered the gene expression levels or mRNA stability that the modified, and thus leading to a decline in lung function and cause lung damage. So the study of smoking cessation intervention in COPD patients on the impact of lung function and with genetic polymorphisms has great significance in China.Objective:To explore the smoking cessation intervention impact on lung function and serum SOD3 concentration for early COPD and the associated with genetic polymorp-hism of SOD3.The aim is to further reveals the importance of smoking cessation inter-vention on early COPD patients.Methods:57 COPD patients with smoking cessation intervention and 35 COPD patients without intervention (the control group) were enrolled in this study. Limosis peripheral blood was taken and whole blood corpuscle genomic DNA was extracted. The genetic polymorphism of SOD3 genes was determined by ligase detection reaction and polymerase chain reaction (LDR-PCR) and serum SOD3 concentration was measured using ELISA.Detected lung function between the two groups before and after intervention by microspirometry.Results:This study observed that:(1)The CC,CG,and GG genotypes were detected in selected objects, but CC genotype of SOD3 were not discovered among three groups.Two sets of frequency distribution had non-discrimination.(2)The FEV1% and FEV1/FVC% were (60.2±17.6,61.7±7.6) and (61.5±8.5,59.9±7.8) in two groups before intervention,they were (67.5±9.3,65.1±5.7) and (59.8±8.4,56±7.2) after 3 monthes. The FEV1% and FEV1/FVC% increased 6.7±1.5and3.8±1.3 after intervention in smoking cessation group. There were significant difference (P <0.05) between intervention group and the control group after 3 monthes. The FEV1% and FEV1/FVC% increased (6.0±0.9,7.1±1.8) and (2.9±1.6,4.1±2.8) in CC andCG/GG genotype three groups about COPD intervention group. There were no significant difference (P>0.05).(3) The serum SOD3 concentration of COPD intervention group and the control were 104.67±91.51U/L and 107.23±72.98U/L before intervention, but 100.82±80.78U/L and 102.74±81.15U/L after 3 monthes, the intervention group was slightly higher than the control group, but with no significant(P>0.05). (4) The serum SOD3 concentration of SOD3 CC genotype carriers was 98.29±87.50 U/L, andl65.50±216.81 U/L with CG/GG genotype carriers. The serum SOD3 concentration of only one SOD3 GG genotype carrier was 570.74 U/L,more than 5-fold compared with the other two carriers.The difference was not significant between two genotype groups (P=0.656) before and after intervention.Conclusions:(1) There were significant difference lung function changes before and ater smoking cessation intervention,and markedly higher than non-intervention group(P<0.05).(2)The different SOD3 genotypes had no significant effect on lung function change after intervention(P>0.05).(3) The serum SOD3 concentration of COPD intervention group was slightly higher than the control group, there was no difference after smoking cessation intervention.(4) The serum SOD3 concentration of SOD3 homozygous mutant GG-type gene was more than 5-fold compared with the other two genotype.However there were no significant difference after intervention(P >0.05).
