The Comparation Of Three Different Surgical Methods Of Varicocele Patients

Objective:The study emphasis on searching for a mini-insulted,safe,and effective modus operandi by comparing with the three operation (high-position small incision of ligation of spermatic vein, laparoscopic high ligation of spermatic vein and ligation of spermatic vein through fold inguen).Contentment:93 cases were varicocele with infertility, 18-36y (min 27y).among the total,35 cases were operated by high-position small incision of ligation of spermatic vein;31 cases by laparoscopic high ligation of spermatic vein;27cases by ligation of spermatic vein through fold inguen.80 cases were sinistro-varicocele;13 cases were diplo-varicocele. All the cases were the single-diseaes.none hasn't be complicated with any other diseaes. Methods:There were 62 cases,which had an operation in our hospital from october 2009 to december 2009,and 31 cases,which had an operation in The third affiliated hospital of Bei Jing University.In the study,there has been four terms:The first term was The high-position small-incision clustered ligation of spermatic vein; The second was laparoscopic high ligation of spermatic vein;The third was ligation of spermatic vein through fold inguen.; The fourth was normal class for comparing. All the cases must check the semen. Only if the semen-check are abnormal and diagnosises varicocele by Doppler,should they to be selected in the study. In order to understanding the semen improvement,They should to rechecte the semen after three month. The length of stay,the cost of treatment,the operated-time and etc. should be recorded in full. Blood rheology and blood gas analysis also should be detected during the operation underway,So that we can compare with the three terms. Results:the first term was shorter than the other two terms in operation time (p<0.05);the first term and the second term hadn't statistically significant in the length of incision (p>0.05);the three terms hadn't obvious difference in The length of stay, the improvement of sperm.the difference,which presenced in the first and third term had statistically significant in the length of incision (p<0.05),but there were no statistically significant in stomach intestine recovery time and the cost of treatment p>0.05). The saturation of blood oxygen, The PaO2 and The HbO2 in spermatic vein were lower than in peripheral vein(P<0.05),especiallythe second term(P<0.01);the others hadn't statistically significant (p>0.05); there were obvious difference between the first and the second term in The saturation of blood oxygen, The PaO2,The PaCO2 and The HbO2in The spermatic vein; otherwise, The saturation of blood oxygen, The PaO2,The PaCO2 and The HbO2in The spermatic vein was no statistically significant between the first and the third term. the three terms have statistically significant in blood rheology;there are not statistically significant between the spermatic vein and the peripheral vein in control term.(p>0.05).Conclusion:The operation of high-position small incision is very popular with varicocele especialy in basic-level hospital.
