| Objective To investigate the prediction value of changes in serum tumor necrosis factorαand interleukin-6 to the progression of disease of patients with compartment syndrome.Methods Select 60 patients of compartment syndrome, According to the clinical performance , the patients were divided into mild group (n=48)and severe group (n=12). Mild group is equivalent to the traditional classification of type I, and severe group compared with traditional classification of type II, III. At 6h after injury and admission 1,3,5,7,10,14,21 d, collected blood samples , changes in levels of serum tumor necrosis factorαand interleukin-6 levels were measured by Enzyme-linked immunosorbent assay (ELISA). Normal control group, 20 cases for the same period in healthy subjects. Operating strictly accordance with the instructions. The muscle preparations from affected extremity were taken at operation and 1,2,3 weeks for biopsy. Using SPSS 10.0 statistical softwa re, Analysing the variance of data. P≤0.05 as statistically significant difference.Results 60 enrolled patients and 20 normal controls were involved in the analysis of results. Mild group and severe group of patients with compartment syndrome of tumor necrosis factorα, interleukin-6 serum concentration compared with the control group were significantly higher. At 3d mo st patients peaked within or close to peak, After that there was a reduction, Tumor necrosis factor-αreturned to normal after 2 weeks,and the IL-6 continued to inc rease more than 2 weeks. The mild levels of serum tumor necrosis factor-αand IL-6 were lower than the degree of severe group of patients,Serum tumor necrosis factor-αand IL-6 levels increased with the severity. Early after injury ,the severe group levels of serum tumor necrosis factor-αin patients was not significantly different from the mild group,and the interleukin-6 levels in patients of severe group were significantly higher than those in mild patients. After 3 days, the severe group levels of serum tumor necrosis factor-αwere also significantly higher than the mild group. muscle pathology of mild group showed partial necrosis, and in the severe group,the pathological specimens of significant necrosis of muscular change was irreversible.Conclusion Dynamic detection of compartment syndrome in patients with peripheral tumor necrosis factorαand interleukin-6 levels may contribute to the assessment of disease,And the dynamic detection of compartment syndrome in patients with peripheral blood levels of interleukin-6 is more useful in predicting changes in compartment syndrome associated with severe complications. |
