| Objective:To verify forsooth effects of standardized oblique-pulling manipulation on lumbar intervertebral disc protrusion through rigorous clinical observation.Method:62 cases of clinical diagnosis are divided into two groups by random through internet. One group is manipulation treatment group with 32 cases, to which oblique-pulling manipulation is used for 20 minutes every other day. The other group is controlled group with 30 cases treated by supine pelvic intermittent traction for 20 minutes everyday (10 seconds interval per 40 seconds sustained treatment). Each group has three weeks'period of treatment. Results:Clinical shows that standardized oblique-pulling manipulation can significantly reduce patients'wrench pain at JOA Score and VAC score, as well as highly improve hoister of straight leg. The cure rate is 78.13%, with a total effective rate 96.88%, which is preceded to traction therapy. The dramatic difference (P<0.05) demonstrates oblique-pulling is superior to traction therapy. The measured protrusion in three-dimensional reconstructed CT image indicates oblique-pulling manipulation is better than traction therapy (P<0.05) by contrast with diameter index and ratio of dimension and high tenth of protrusion. Conclusion:Clinical studies have demonstrates:1. Standardized oblique-pulling manipulation is far in advance of traction therapy in melioration of symptoms. 2.Standardized oblique-pulling manipulation can reduce protrusion and release pressure of ganglion, which is greatly better than traction therapy as well.3. It also confirms the criterion and mechanism of standardized oblique-pulling manipulation. As for remodel of protrusion shape in biomechanical aspect and its long-term effect remains to be further studied. |
