Objective: Through electrocardiogram exercise treadmill load test to evaluate patients with type 2 diabetes, and according to heart rate, blood lactic acid of the motion to determine the best exercise intensity, finally to guide patients for safe and effective personalized exercise therapy. In heart rate table supervision, patients with type 2 diabetes proceed the number 4.2 diabetes quantitative exercise movement and the same exercise walking movement,comparing the blood glucose and blood pressure changes before exercise and after exercise the 60min 10min, 30min,for patients to choose appropriate mode of motion, provides the basis.Methods: (1) motion of sports load test for safety evaluation: we choose 86 patients with type 2 diabetes in our hospital from December 2009 to April 2011.They are required for electrocardiogram exercise treadmill testing, and the bicycle is installed in power 40r/min and setted the power to increase every 2 min 20W, at the end of each power levels plasma lactic acid is detected,and motion detection process of 12-lead league electrocardiogram and blood pressure automatically are recorded. (2)consideration of blood glucose immediately after treatment: we choose 60 patients with type 2 diabetes of outpatient or community clinic from June 2009 to December 2010.They participate in different time points of three kinds of activities, including the first day for test quiet condition, the next day for prescription exercise and the third day for treadmill. Adopt itself controlled trials, patients are observed before and after these three ways of blood sugar at different time points immediately.Results: (1) motion of sports load test for safety evaluation: 86 cases of type 2 diabetes in ECG exercise treadmill testing,when they reach to the power of 100W and 120W, their blood lactic acid respectively are (3.75±1.93)mmol/L和(4.83±2.18)mmol/L.According to the standard of blood lactate threshold,their target heart rate are respectively (123.30±19.52)131.70±19.52bpm. AT the end of the test their ratings of perceived exertion is (15.79 + 1.78) points. The correlation coefficient of blood lactic acid and power, heart rate and power, heart rate and blood lactic acid respectively are r= 1.000, r = 0.943 and r = 0.983. Results for the ECG exercise treadmill testing,there are 6 patients diagnosed for positive. Among of them,there are 2 cases already diagnosed with stable coronary heart disease before exercise,the other 4 cases are without cardiovascular disease clinical manifestation. Negative groups and positive group of the heart rate are compared statistically significant differences when their power reach to 100W (P < 0.05). Positive and negative groups of the PRE at the end of test respectively (15.33±3.01)points and(15.87±1.64)points(P > 0.05). Negative groups of heart rate,lactic acid, systolic pressure and diastolic pressure increases linearly with increasing power ; positive group curve lower than negative groups, and the blood pressure of positive group has dropped trend when athletic end. (2) movement for immediate glucose role after treatment of research: quiet group, prescription group, running group of glucose are extended over time declined (P < 0.05). Prescription group and running of unit movement speed after 60min and 30min respectively compare with 10min has statistically significant difference (P < 0.05).Prescription group of blood sugar descending velocity is diminishing trend, and running group only in 60min showes declines. Running units before and after exercise the relative change rate (P heart rate) and the relative changes before and after exercise blood sugar (P blood sugar) is inversely related (P < 0.05), and prescription group P heart rate and P blood sugar is not related to (P < 0.05). But the SBP before and after exercise prescription groups was statistically significant comparative differences (P < 0.05), running units of before and after exercise SBP comparative differences have statistically significant (P < 0.05).Conclusions: (1) Through the ECG monitoring of sports load test, the heart rate, blood lactic acid and power are highly relevant relations.Using the heart rate and blood lactic acid to determine the exercise intensity is scientific and reasonable. It is not difference whether using the lactic acid threshold or heart rate percentage for best sport intensity,but the former calculation result is more accurate.(2) ECG exercise load test can found diabetes patients combined cardiovascular disease. In the same exercise intensity, the heart rate level of sports experiment positive patients is lower than negative groups.The two groups of blood lactic acid level reaction consistent with athletic intensity.(3) In the same exercise intensity, prescription group and running units can reduce blood glucose, and they are consistent about impacting the level of their blood glucose role.Prescription group of blood glucose decreasing speed is gradually diminished within 60min after exercise.And the run units is still not declined in 30min after motion.The two group of of blood glucose are restore to resting levelstill no decline in the two groups of blood glucose decreasing speed in 60min after motion.(4) Different intensity exercise has different effect on systolic blood pressure and diastolic blood pressure.The high intensity exercise can descend diastolic blood pressure after movement.And moderate intensity exercise the effect of lower systolic blood pressure.Different way of exercise has slight affecting blood pressure in different exercise intensity.
