Medical quality is not only the fundamental guarantee for existence and development of hospital, but also is a constant theme of hospital management. With the sustained progress of improving living standard in various undertakings and the continuous deepening of medical health reform, the enhancement of medical quality construction has increasingly shown its importance and urgency.Jilin Oilfield General Hospital is a three level of first-class hospital. After years of development, the hospital's software and hardware conditions have improved continuously. Medical business trend has continued to grow for the past five years. The indicators of reflecting the quality of medical quality remain at a high level.To fully understand the current situation of medical quality management in Jilin Oilfield General Hospital, the discussion group took the organization system of medical quality management and the different components of the quality status in the process of out-patient, emergency and in-patient as key points of investigation. They investigated the present situation of hospital quality management organization and carried out satisfaction surveys of patients. Especially, 1040 hospital medical records of 25 clinical departments in 2010 were taken for the surveys of the completion of the medical system three rounds, surgical treatment management cases, and blood transfusion management.Research has revealed that Jilin Oilfield General Hospital has already set up the basic framework of medical quality management organization, but some questions still exist in the basic framework, such as the duplication of the functions of some of the quality management department with other departments, unclear responsibilities, and poor quality control functions of quality management departments at various levels. The link quality status is good. Discharged patients'satisfaction is high, but they are overall not satisfied with the long time to wait for treatment, which indicates that there are unreasonable problems existing in the process of out-patient clinics. In addition, the core medical system has not been practically implemented during the process of in-patient cares. Survey also reveals that there are still shortcomings existing in the medical quality management including unsound organizational structure and rules and regulations, ineffective link quality control, a higher rate of repeated quality problems, and the low efficiency of quality management. In response to these problems, one should first establish the concept of TQM (Total Quality Management) and take the comprehensive medical quality management philosophy as an important foundation and a basic way to carry out quality management.In general, the construction of medical quality management system of Jilin Oilfield General Hospital should be focused on the demand-side of health care services and should be based on a continuous improvement principle. The medical quality management objective taking into full consideration the needs of demand-side of health care services should include safety, effectiveness, accessibility, appropriateness, health care demand-side participation, efficiency and so on. Based on the above principles, a comprehensive medical quality management system should be built within the whole hospital that includes the organization system, standard system, monitoring and evaluation system, support system, and other sub-systems; meanwhile, each sub-system should contain a number of specifications and requirements. In this way, a three-dimensional mesh-style comprehensive medical quality management system could be formed. To be specific, the related work should be mainly carried out from the following facets: i) improving medical quality management organization, specifying institutional responsibilities, and strengthening quality management department organizations; ii) establishing a complete medical quality standardization system including basic quality, process quality and terminal quality standards; iii) implementing full case management and optimizing business processes, iv) enhancing disease quality control, department quality control and monitoring summary, feedback, and analysis, thus effectively control of medical defects; v) strengthening the quality cultural development, regulations construction, information systems development, and education and training.The building steps of medical quality management system should include formulation of medical quality principle, setting of medical quality goal, the process analysis and preparation of medical quality management system documentation. To guarantee the effective operation of medical quality management system, one should organize the whole staff participation, implement management by objectives, improve information communication and feedback, and establish review mechanisms.
