Objectives: There are controversial opinions for the preferred treatment of early glottic cancer. Through this study, the author wants to provide some bases for the selection of early glottic cancer treatment.Method: Use the electronic retrieval database, such as MEDLINE (1966–2010), EMBASE(1984–2010), Cochrane Library,CBM(1979–2010), CNKI(1994–2010), Wanfang Database, etc. Meta-analyses are performed by RevMan 5.0 software. Results: In XRT vs TOL's study, there is significant heterogeneity in the documentations about the complications. So Meta-analysis is given up during this study. For the recurrence and 5-year locoregional control, XRT group has no significant statistics difference with TOL group by eliminating the heterogeneity or using the subgroup analysis. But the results still show that there is less recurrence and more locoregional control in TOL group than those in XRT group. On the aspects of death of disease, overall survival, and laryneal preservation, there are small heterogeneity differences in the documentations. So after comprehensive research, we get the result, that is using TOL has more advantages compared with XRT. In the XRT vs SUR study, there are small heterogeneity differences in the documentations on the aspects of the recurrence rate, the rate of death of disease, the 5-year overall survival rate, and the 5-year locoregional control rate. So after comprehensive research, we get the result, that is the traditional surgury(SUR) has more advantages than external radiation(XRT). For laryneal preservation, the documentations has more significant heterogeneity. By using subgroup analysis, there are no objective differences between XRT and SUR. On the aspects of the complications, we need further studies to make sure if the evidences are credible. Conclusions: TOL could be completed in the clinic and the cost was much more cheaper than XRT. So TOL has prominent advantages and it could be used as the first choice for the treatment of early glottic carcinoma. But for the patients who have larger tumors that even affect the anterior commissure, there was no clear conclusion for the treatment. Based on documentations, traditional surgery is the perfered choice. Because the data included in this study. We recognize that the datas included in our study are retrospective ones, so we need more scientific and precise research data to get a more scientific conclusion.
