Objective:To evaluate the value of transoral laser microsurgery in patients with glottic carcinoma,and to explore the causes of recurrence of early glottic carcinoma after transoral laser microsurgery,so as to guide the clinical application and promote the application of transoral laser laryngoscope.Methods:From December 2010 to October 2017,69 patients with glotticlaryngeal carcinoma were treated by CO2 laser microscopy under laryngoscope.All the69patients were confirmed glottic laryngeal cancer by tumor bite test,and were treated by laryngeal microsurgeryunder laryngoscope.The resected specimens were sent to pathological examinationafteroperation,allofthemwereconfirmedas laryngealcancer patients.The secondary hospitalization was observed,and 11of them were considered as recurrent laryngeal cancer patients afterdiagnosis.Objective:to investigate the causes of recurrence of glotticcarcinoma by investigating the invasion,t-stage,laser operation and body mass index of the patients with glottic carcinoma during the first operation.Results:A total of 69 patients with glottic laryngeal carcinoma were studiedin this study.11 patients had postoperative recurrence in situ,the recurrence rate was 15.94%.Among them,18 cases had tumor invasion and 6cases had recurrence,the recurrence rate was 33.33%.Tumor T stage:7patients with Tis stage,including 2 cases recurrence,T1a stage of 47cases,including 4 cases recurrence,T1b stage of 9 cases,including 2 cases recurrence,T2 stage of 6 cases,including 3 cases recurrence.The difference between the two groups was statistically significant?P<0.05?.There were 39 patients with BMI>24,5 of whom relapsed.Tumor invasion and pathological T stage are the factors of recurrence of glottic carcinoma after operation.The operation method and body mass index predict the possibility of recurrence after operation.Conclusion:1.Glottic laryngeal carcinoma was treated by laser microsurgery,but the recurrence rate was still higher than that before the invasion of glottic carcinoma.Preoperative cervical CT,cervical lymph node color Doppler ultrasound and other related examinations should be improved to understand the extent of tumor invasion.2.The higher the T stage,the higher the recurrence rate.3.There was no significant correlation between body mass index and recurrence of glottic carcinoma after oral laser micro-treatment.A further large number of this comparative tests are required.
