Thymectomy For Myasthenia Gravis Patients And Related Factors Of Efficacy

Objective:To investigate the influence factors of thymectomy for myasthenia gravis. Methods:MG patients from the First Affiliated Hospital of Fujian Medical University during January 2005 to December 2010 were devided into surgical(50 cases) and non- surgical group.Two groups were paired as 1:1 type under those condictions age,gender and starting symptom or Ossermann type.Efficacy was judged as valid or invalid. Measurement datas were compared using paired t test.Count data were compared with Chi-square test or Fisher exact test.Using Binary Logistic regression to analysis the comparison in operation,gender,starting symptom, thymic abnormalities, disease type, thyroid disease,using other treatments,age and crisis.Rusults:All 50 patients in surgical group and 46 patients in non-surgical group were followed up.As a result, 46 matching pairs were made. In surgical group, the cure rate was higher in non-thymoma than thymoma(P=0.039).The efficacies in different ages were significantly different (P<0.05).It was different in the distribution of the thymoma in age (P<0.05).There were no differences between surgical and non-surgical group in the age of onset,the steroid dose and the course of disease(P> 0.05).In the≥40years age the efficacy of thymectomy between thymoma group and non-thymomas were statistically significant(P<0.05). According to the Binary Logistic regression, crisis is an independent risk factor,and start age is a protect factor.Conclusion:It is common that the thymus is generally abnormal in MG patients, thymus hyperplasia is mainly occurred in age less than 40years,while thymoma is higher incidence in the others.The efficacy for thymectomy needs to be proved by enlarge sample size.If one patient is diagnosed with myasthenia gravis upper 40years old,thymectomy is prefered. Crisis is a independent risk factor,while older than 40years old is a protect factor.
