Objective : To study atorvastatin′s intervention to blood pressure variability in elder essential hypertension patients.Methods : This is an aperspective,random, parallel clinic study. According to WHO diagnosis criteria of hypertension, 80 subjects with essential hypertension(EH), who have been treated with amlodipine(5-10mg qd)and the blood pressure had been controlled to the target ,were included and divided into two groups:statins group and the control group. The statins group(n=40) received atorvastatin 20mg per night besides amlodipine. While the control group(n=40) received amlodipine (5mg qd) alone. All followed up for 24 weeks. Ambulatory blood pressure mornitoring, blood glucose and lipid level were measured at the baseline and 24 weeks of treatment respectively. Averaged 24-hour ambulatory BP(24SBP and 24DBP) ,BP variability were evaluated by the SD of the averaged 24-hour ambulatory BP recordings(24SBPV and 24DBPV),5:00-8:00Am morning blood pressure surge(MBPSV of 5:00-8:00AM),and average real variability in 24-hour ambulatory BP recordings(ARV24).Results:1.At the baseline, there was no significant difference between the statins group and the control group(P﹥0.05).2. 24SBP, 24DBP,24SBPV, 24DBPV of the statins group and the control group at the baseline were 133.11±17.42 vs 132.91±17.7,69.71±12.75 vs 68.94±13.30,13.20±3.10 vs 13.12±2.91,8.71±2.1 vs8.88±2.31,respectively(P﹥0.05);significant difference were showed after 24 weeks follow up,125.82±16.59 vs 127.99±16.76,66.39±10.90 vs 66.63±12.11,12.14±2.42 vs 12.56±2.64,8.43±2.01 vs 8.67±2.25, respectively(P﹤0.05).3. MBPSV of 5:00-8:00AM of the statins group and the control group at the baseline were 24.70±12.28 vs 23.78±14.52,respectively,and it had no significant difference (P﹥0.05).After 24 weeks of therapy, they were 22.86±11.77 vs 22.89±11.52,and it had significant difference(P﹤0.05).4. ARV24 of the statins group and the control at the baseline were 10.12±3.11 vs 10.38±2.67, respectively (P﹥0.05).while, it's no significant difference after 24 weeks of therapy(10.06±2.34 vs 10.14±2.37, P﹥0.05).5. The lipid levels including TC,TG,HDL ,LDL of the statins group and the control group were 4.05±1.07 vs 3.96±0.95, 2.09±0.95 vs 1.98±0.79,1.03±0.29 vs 1.04±0.27, 2.04±0.71 vs 1.93±0.59 at the baselin respectively (P﹥0.05). After 24 weeks of therapy, TC,TG,LDL was decreased significantly in the statins group ( 3.53±0.89 vs 3.77±0.75,1.65±0.75 vs 1.68±0.56,1.79±0.63 vs 1.84±0.49,(P﹤0.05),while HDL was 1.08±0.23 vs 1.09±0.24 , and it was increased after treatment but significance(P﹤0.05),No changes of the blood glucose level were detected in both groups (P﹥0.05)Conclusion : Atorvastatin can further improve the blood pressure variability of elder essential hypertensive patients whose blood pressure have been controlled to the target with amlodipine.
