Compared Study On The Different Expression Of PGC, PTEN, HER-2, CEA And CA72-4 Between Mycotic Gastric Ulcer And Ulcerated Gastric Adenocarcinoma

Objective:To investigate and compare the expression of five tumor markers including PGC, PTEN, HER-2, CEA and CA72-4 in mycotic gastric ulcer and ulcerated gastric adenocarcinoma in order to explore their clinical pathological value for differential diagnosis of mycotic gastric ulcer and ulcerated gastric adenocarcinoma.Methods:Sixty-six paraffin specimens, which were pathologically confirmed their diagnosis including 31 mycotic gastric ulcer and 35 ulcerated gastric adenocarcinoma, were collected from surgical resection or gastroscopic biopsy from March 2002 to December 2009 in the second Xiangya hospital of central south university while 10 specimens of normal gastric tissue were used as the control. Expression of these five tumor markers including PGC, PTEN, HER-2, CEA and CA72-4 was detected by Elivision immunohistochemical technique in the whole 76 samples. The positive cells were counted under optical microscope and the data was statistically analyzed byχ2 test and ROC curve with SPSS 13.0 software for windows.Results:Expressive positive rates of PGC, PTEN, HER-2, CEA and CA72-4 were 67.7%,93.5%,9.7%,16.1%,29% respectively in mycotic gastric ulcer while those in ulcerated gastric adenocarcinoma were 25.7%, 68.6%,57.1%,45.7%,65.7% respectively; But the expressive positive rates of the above five tumor markers were 100%,100%,0%,0%,20% respectively in normal gastric tissue. The positive rates of PGC and PTEN in ulcerated gastric adenocarcinoma were significantly lower than those in mycotic gastric ulcer (P<0.05). But the positive rates of HER-2, CEA and CA72-4 in ulcerated gastric adenocarcinoma were significantly higher than those in mycotic gastric ulcer (P<0.05). Although the expressive positive rates of PGC and PTEN decreased while those of HER-2, CEA and CA72-4 increased in mycotic gastric ulcer compared with normal gastric tissue, there was no significant differences between mycotic gastric ulcer and normal gastric tissue (P>0.05).Conclusion:1. That the positive rates of PGC and PTEN were significantly lower and those of HER-2, CEA and CA72-4 were significantly higher suggested that detection of PGC, PTEN, HER-2, CEA and CA72-4 is useful for the differential diagnosis of ulcerated gastric adenocarcinoma and mycotic gastric ulcer.2. PTEN was the most sensitive and accurate marker for the differential diagnosis of mycotic gastric ulcer and ulcerated gastric adenocarcinoma among the five tumor markers while HER-2 was the most specific one.3. Combined detection of five tumor markers including PGC, PTEN, HER-2, CEA and CA72-4 could be benefit for the clinical pathological differential diagnosis between mycotic gastric ulcer and ulcerated gastric adenocarcinoma in order to increase diagnostic rate of ulcerated gastric adenocarcinoma. The most efficient ones were PGC and HER-2 in two-marker combination, or HER-2, CEA and CA72-4 in three-marker combination, or PGC, PTEN, HER-2 and CA72-4 in four-marker combination.
