Follow-up Studies Of 140 Gastric Mucosal Epithelial Dysplasia Patients

Background and Objective:Gastric mucosa epithelial dysplasia is an important precancerous lesion of gastric cancer. Studying dysplasia plays an special role in preventing and treating gastric cancer. Our study aims to analyze the endoscopic demonstrations of gastric dysplasia, whether accompanied by atrophy or intestinal metaplasia, outcomes and its relationship with eradication of Helicobacter pylori, to investigate rational follow-up plans.Methods:We collected patients who had received endoscope examines and been diagnosed epithelial dysplasia by biopsies in our endoscopy center. Digestive Department, the 2nd Affiliated Hospital of Zhejiang University, School of Medicine from January to December in 2008. They periodically received endoscope examines to trace the changes of dysplasia. And the endoscopic demonstrations, whether accompanied by atrophy or intestinal metaplasia, the relationship with eradication of Helicobacter pylori, ways of treatments and the first follow-up interval were our analyzing points.Results:Among 140 cases, there were 126 mild,13 moderate and 1 severe dysplasia patients; a variety of endoscopic demonstrations of gastric mucosa epithelial dysplasia were found in our analysis, between which, erosion and atrophy were the most common; Data showed that 72.14%(101/140) of all the patient were accompanied by atrophy, and 78.57%(110/140) with intestinal metaplasia; 60 cases received eradication of Helicobacter pylori, and 42 of them succeed; Among the mild dysplasia patients, 88.10%(111/126) regressed,9.52%(12/126) remained unchanged,2.38%(3/126) progressed to severe stage or early cancer: while in moderate group, dysplasia disappeared in 61.54%(8/13). kept persistent in 7.69%(1/13). and became severe in 30.77%(4/13) of the patients; 4 cases received endoscope treatments, and the pathological results were mild, moderate, severe dysplasia and early gastric cancer, respectively; 70.63% of the mild dysplasia patients and 69.23% of the moderate dysplasia patients received endoscopic follow-up within the first year after the diagnosis, and 20% of all the patients got re-examined within 3 months.Conclusion:All the patients who have been diagnosed of dysplasia should receive endoscope examines and biopsied periodically; Most of the mild dysplasia can be reversed, while the follow-ups of moderate dysplasia should be strictly monitored, and patients should receive endoscopic treatments or surgery if they were diagnosed of severe dysplasia.
