Objective To evaluate the epidemiological characteristic and risk factors of chronic obstructive pulmonary disease(COPD) in Kunming city.Methods Using the same protocol and questionnaire, performing cluster-random-sampling survey for the population aged over 40 years in Kunming city. The post-bronchodialators forced expired volume in one second to forced vital capacity ratio (FEV1/FVC)×100%<70% was used as diagnostic criteria of COPD.Result (1)Among 2503 sampling subjects,2193 participants completed the questionnaire and spirometry (response rate,87.0%),1085 males and 1108 females. The mean age of the total study population was 57(SD, 11.81) years, The population distribution are:Xishan area (535 subjects, 24.4 percent), Guandu area (599 subjects,27.39 percent), Panlong area (275 subjects,12.5 percent), Wuhua area (784 subjects,35.8 percent). The Han ethnic group is in a great measure.(2) The crude prevalence of COPD was the highest in Xishan area (5.8%) and lowest in Guandu area (2.0%). The overall prevalence of COPD was 4.2%. The prevalence of COPD was significantly higher in men than in women (6.2% vs.2.3%, P<0.001) and increased with age (Ptrend<0.001).(3) COPD patients are not well educated with knowledge about COPD, few people have been tested with spirometry, therefore COPD education should be enhanced and various measures should be taken to improve the treatment results. (4) Multivariate logistic regression analysis showed that age, sex, smoking, cooking, exposure to occupational dusts, decorating living room, family history, worse ventilation in kitchen room are risk factors.Conclusion The overall prevalence of COPD was 4.2%. The prevalence of COPD in Kunming city is lower than average level of the whole country and the patients have poor knowledge about the disease. The preval ence of COPD related with sex, age, smoking history, family history, cooking, exposure to occupational dusts.
