A Survey Of "go West" Country-level Doctors Training In Yunnan Province

[Objective]To known the doctors' basic backgrounds who participate in the training of the Ministry of Health departments' "go west" physicians at country level in our province and to understand what is their training demands or training needs. To find out the current situations and the needs of our county-level doctors' training, continuing medical education, and general practice education. We survey and supervise the working implementation of the Ministry of Health departments' "go west" physicians at country level in our province. According to our survey we giving summers of our training and summing up experiences, improving our training methods, assessing the results of our training rectifications and improving our training level continuously.[Methods]Review references in and abroad, we making questionnaires and giving them for the doctors who take part in the third to the fifth number of our province "go west" doctors' training at country level. The third number is for the physicians and the pediatricians, the fourth number is for the gynecologists and surgeons, and the fifth number is for the medical imaging. Before the training we survey the doctors' basic situation and the needs for the training. In the middle term of the training we survey doctors' satisfactions and the hopes for correction our training methods. In the end of the training we survey doctors' satisfactions to. At the same time we sampling part of our students randomly and give them deep interviews. We sampling some of our students' hospitals to visit and survey the leaders, some of our students' partners to deep interview. Some of our students' partners who are not visited are surveyed by using telephones. The database we using is Epidata 3.02, to exam it and using SPSS11.5 for analyzing. The main methods of our statistical are descriptive analysis, and the qualitative indicators are using rate, ratio, statistical tables, and statistical graphs. The analyzing of our date are Ridit analysis, Chi-Square test, K-Related sample test. [Results](1) Results for the basic backgrounds, situations and the needs for training before their training:201 doctors in the survey of our program, there are 108 males(53.7%)and 93 females(46.3%), their aging group are mainly in 20 to 30 age groups and in 30 to 40 age groups. The students in our three classes feel that it is very important to training country-level doctors (total R=0.5000) and have no significant difference between the three classes (F=0.318, P-0.728> 0.05). The participants considered that their current medical knowledge are lack and still have difficult to mach country-level business requirements(total R=0.6664) between the three classes there are have not statistically significant (F=0.000, P=1.000).Students are always want to attend the training and their training are always based in the Ministry of Health departments. The mainly reason for the doctors not to take part in the training is too busy to work. The theme of the training needs for our students have statistically significant (x2=302.268, P=0.000). The mainly theme is to improve the diagnosis and to improve the capacity of treatment needs (mean rank is 1.84), to improve their theoretical (mean rank is 2.32), explore new ideas (average rank is 2.92), to raise professional ethics (mean rank is 3.81) and access to education degree (mean rank is 4.12). Recently, the doctors who participate in our training have statistically significant (x2=37.194, P=0.000), our class the students for the physicians and the pediatricians needs to strengthen and enrich the expertise of the top three are internal medicine, pediatrics and basic medical operations, while the class for the gynecologists and surgeons needs to strengthen and enrich the expertise of the top three are surgery, basic medical operations, and internal medicine, but for the medical imaging class needs to strengthen and enrich the expertise of the top three are basic medical operations, internal medicine and surgical.(2) Results for the medium-term of our survey in the training:There are have no difference statistically significant between the satisfaction of training text book between our three class students (total of R is 0.7550, F=0.000, P=1.000). The objects of our training have a good matching with the latest developments in the disciplines (total R is 0.5000) there have there level between the three classes, the class for the physicians and the pediatricians, medical image science class, and the class for the gynecologists and surgeons (between the class for the physicians and the pediatricians and the gynecologists and surgeons P=0.000, between the class for the physicians and the pediatricians and medical image science class P=0.000, and between the class for the pediatricians and medical image science class P=0.011). The object of Training teaching content and county-level physicians in practice are more in line with the needs of work (total of R is 0.5000), the class for the physicians and the pediatricians is better than the class for the gynecologists and surgeons (P=0.005). The doctors are very satisfied with the plan of the teaching, the class for the physicians and the pediatricians is better than the class for the gynecologists and surgeons (P=0.010), and the medical image science class is better than the class for the gynecologists and surgeons too (P=0.000). Three of our classes have a larger harvest (total of R is 0.5000, F=1.073,P=0.344). Students have a high level in improving clinical capacity, explore new ideas, and improve their theoretical levels, but have lack of scientific capabilities.(3) Results for the end of our survey in the training:The students of our three classes are satisfied with our training (total of R is 0.5000), the class for the physicians and the pediatricians are better than the class for the gynecologists and surgeons (P=0.000), and the medical image science class have better satisfied than the class for the gynecologists and surgeons (P=0.000). The students are hoping to training them in medical Universities (P=0.004). More and more doctors want training 6 months after the training (P=0.00) and few of them select lyear (P=0.00). After the training three of our classes have a good clinical improvement analysis (total of R is 0.5050), the classes for the physicians and the pediatricians and for the medical image science have better clinical improvement analysis than the class for the gynecologists and surgeons (P=0.000).(4) Results form the leaders of students' hospitals and the doctors' workmates: the leaders took great importance for county level doctor training, and made some measure to support the training. They are very satisfied with the doctors' clinical ability when the doctor came back to done their work and they promise to send more of their workers to take part in the training. The workmate of the students are satisfied with the training and had high assessments, they want to have chance to take part in the same training.[Discussions and recommendations]According to our survey we found conclusions as follow:(1) The doctors who take part in the Ministry of Health departments' "go west" physicians at country level in our province are the basic physician in the country. Their current knowledge and technologies can not mach with the needs of country hospital business, they need to improve their clinical thinking and professional ability urgently. (2) We have achieved certain results in the country level training in our province, the participants of the training are full of confidence; different classes have differ needs of their training. Their major needs are their working field, we should give them training to their working field and the training should be combined with the actual conduct of county hospitals characteristic. (3) The students have little satisfaction of our text book for country-level doctor training. The class for the physicians and the pediatricians achieved higher satisfaction about our training, our teachers should sharing experience summarize their success. (4) The students in our three classes felling 3 months training are too short,6 months is the best training times.
