【Objective】To find a relationship among the clinical, pathological morphology and morphological indexes and a judgment of the differences and similarities between adult IgA Nephropathy (IgAN) and children IgAN and between children IgAN and children Henoch-Sch?nlein purpura nephritis (HSPN).【Methods】30 cases of adult IgAN,30 cases of children IgAN and 30 cases of children HSPN were collected, recording the dates of gender, age, duration of time, systolic pressure, diastolic pressure, blood urea nitrogen, serum creatinine, albumin, 24h proteinuria. According to the immunoglobulin deposition, the IgAN and HSPN had 4 typing. the value density, area density, average optical density and integral optical density of deposition of pathological examples were analyzed by computer imaging analysis system of Motic Med 6.0 after routine staining and immunohistochemistry staining, observing the pathology indexes, including mesangial proliferation, cellular crescents, cellular fiber or fibrous crescent, endothelial cell proliferation, necrosis, glomerulosclerosis, interstitial inflammatory cell infiltration, interstitial fibrosis, renal tubular multifocal atrophy, renal vascular lesions.【Results】Part I: Comparison of adults IgAN and children IgAN image analysis and pathomorphologyCourse of the disease, systolic pressure, diastolic pressure of adult IgAN were significantly higher than children IgAN. According to the immunoglobulin deposition, both of the adult and the children with IgAN were the A + M type and A + G + M type accounted for majority. Both of the value density, area density, average optical density and integral optical density have statistical significance. According to classification, many of adult IgAN were gradeⅢand gradeⅣ, while children IgAN were gradeⅡand gradeⅢfor easily seen. Mesangial proliferation, interstitial inflammatory cell infiltration, necrosis, chronic crescent, glomerulosclerosis, interstitial fibrosis, renal tubular multifocal atrophy, renal vascular lesions of adult IgAN were significantly higher than children with IgAN.Part II: Comparison of children IgAN and HSPN image analysis and pathomorphologyAge of disease onset was statistical significance between children IgAN and HSPN. According to the glomerular deposits, the children with IgAN and HSPN were based on A + M-type and A + G + M type. The value density, area density, average optical density and integral optical density had statistical significance in image analysis of sediments. Many of children IgAN, according to WHO classification, were gradeⅡ, while many of children HSPN, according to ISKDC classification, were gradeⅢ. Comparing with IgAN, the frequency of endothelial cell proliferation, necrosis were much higher in HSPN, By further analysis, the cellular crescents, endothelial cell proliferation, necrosis and chronic crescent between those two diseases also had statistical significance.【Conclusion】1. Comparing with children IgAN, the course of adults IgAN was much longer, chronic pathological changes much more significant, renal damage much more serious2. The value density, area density, average optical density and integral optical density of deposition positive correlation with age in IgAN.3. Age of disease onset of children HSPN was much more concentrated than children IgAN.4. The average optical density of children HSPN was much higher than the children IgAN. However, it had a lower integral optical density than the children IgAN.5. The active lesions were easily seen in Children HSPN.
