Objective To study the early diagnostic value of IP-10 in preterm infants with bacterial infection.Methods 1. Objects:74 preterm infants admitted to our NICU from 2010.6 to 2011.2 were enrolled in this study. They were divided into two groups: 35 infants in infectious group and 39 infants in non-infectious group. 2. Research methods:1) Data collection:to record the style of delivery,gender,gestation age,birth weight,age and clinical manifestation. 2) sample test:The blood sample of preterm infant suspected infection was collected within 24 h. IP-10 was measured by ELISA. WBC,PLT,hs-CRP and the percentage of neutrophil CD64 were also measured at the same times. Blood culture was done when sepsis was suspected. 3.Data analysis: The datas were analyzed by SPSS 13.0 software. Variables between the two groups were tested by using the chi-square test for categorical data and the Mann-Whitney U-test for numerical data. Receiver-operating characteristic(ROC) curves were constructed to illustrate IP-10,hs-CRP land CD64% levels by software. Sensitivity,specificity,positive and negative predictive values were calculated for the cut-off point that represented the best discrimination as derived from the areas under the ROC curves.Results1. The WBC counts and PLT counts are not significantly differences between the infectious group and the non-infectious group.(P>0.05).2. The levels of IP-10,hs-CRP,CD64%in the infectious group are higher than those in the non-infectious group. There are significantly differences between the two groups(P<0.01).3. The sensitivity and speciality of IP-10 level are higher than those of hs-CRP,CD64%.4. Positive and negative predictive values of IP-10 are higher than those of hs-CRP,CD64%.Conclusion1.WBC and PLT are not good predictors in distinguishing between preterm infants with and without infection.2.hs-CRP,CD64% have diagnostic value in preterm infant with bacterial infection,but their sensitivities are lower than that of IP-10.3.IP-10 is possiblly a sensitive early maker of infection in preterm infant...
