Objective:To investigate the level of thrombopoietin (TPO) and C-Mpl in systemic lupus erythematosus (SLE), and the relationship between TPO and other clinical or laboratory findings. In order to evaluate the clinical importance of TPO in the pathogensis and treatment of SLE patients with platelet decrease.Methods:111 SLE patients were grouped by blood platelet counts (BPC),45patients with thrombocytopenia,66 patientswith normal platelet,46 healthy people as the control group. Serum levels of TPO were tested by enzyme-linked immunosorbent Assay (ELISA) in all patients and controls.39 SLE patients and 20 control group were tested the rate of TPO receptor (C-Mpl) expression by Flow cytometry (FCM). according to SLE disease active index (SLEDAI), we devided SLE patients into active group and non-active group, to see the difference of the C-Mpl expression between the two groups. To analyze the relationship of C-Mpl and the clinical laboratory features of SLE patients. megakaryocyte were count by bone marrow puncture with26 SLE patients,the rate of C-mpl was tested by FCM in the bone marrow.Rssult:1. The level of serum TPO in SLE patients was higher than healthy controls(P<0.05), patients with thrombocytopenia were higher than patients with normal platelets, but there were no statistical significance (P>0.05).2. No positive relationship was found among TPO,platelet counts and C3 (r-0.053,P= 0.57>0.05; r=-0.142,P=0.137>0.05).There were positive relationships among theconcentration of serum TPO and erythrocyte sendimentation rate (ESR),C-response protein (CRP) and SLEDAI (r=0.218,P=0.021<0.05; r=0.342, P=0.001<0.05;r=0.225, P=0.018<0.05)3.The expression of C-Mpl inSLE patients was higher than healthycontrols(P 0.05),but there were no statistical significance between SLE patients with thrombocytopenia and patients with normal platelets(P>0.05).4. No positive relationship was found among C-Mpl and clinical,laboratory features of SLE patients(P>0.05).But there were positive relationship between the concentration of Serum TPO and the expression of C-Mpl (r=0.379, P=0.030<0.05).5. Megakaryocyte count decrease in the SLE patients and major infantility type, the rate of C-Mpl in patients with thrombocytopenia were higher than patients with normal platelets.Conclusion:1. The level of serum TPO and the expression of C-Mpl in SLE patients were higher than healthy groups.which suggested TPO/C-Mpl Had taken part in the pathology of SLE with thrombocytopenia.2. There were positive relationships between the concentration of serum TPO and ESR,CRP, the level of TPO might influenceedby immunological and inflammatory reaction, not simply the activity of the SLE disease.3. No positive relationship was found among C-Mpl and clinical,laboratory features of SLE patients, but there were positive relationship between the concentration of serum TPO,the expression of C-Mpl, we can conclude that the C-Mpl expression was mainly regulated by the level of serum TPO.4. megakaryocyte count decrease in the SLE patients and major infantility type, Prompting that there were Immature disorder in the SLE patients.
