ObjectiveThrough observing the biological quantitative targets microvessel density(MVD),lymphatic vessel density( LVD)'s relationship between the group of non-lymph node metastasis and the group of lymph node metastasis in peripheral non-small cell lung and the correlation between nonsmall-cell lung cancer multislice spiral computed tomography(MSCT) perfusion parameters blood volume (BV) and permeability (PMB) and the expression of tumor microvessel density(MVD),vascular endothelial growth factor(VEGF),lymphatic vessel density(LVD),vascular endothelial growth factor-C(VEGF-C).To investigate the value of judging tumor's aggressiveness, directing the clinical cure and assessing the prognosis of these patients by the method of nonsmall-cell lung cancer multislice spiral computed tomography(MSCT) perfusion imaging.Methods40 cases of NSCLC which pathological type have been proved by surgery were collected. And MSCT perfusion imaging with 128 spiral CT equipment was conducted to obtain perfusion image and perfusion parameters including blood volume (BV) and permeability (PMB) for all patients before surgery. Clinical data and pathological data were completed for all patients and lung cancer patients didn′t undergo any therapy before perfusion. The expression results of MVD, VEGF,LVD,VEGF-C in lung cancer by immunohistochemistry were observed and were set up two groups by mediastinal lymph node metastasis. The statistic analyses were completed when all data were obtained. The comparisons of expression of MVD and LVD between the group of positive lymph node metastasis and the group of non-lymph node metastasis were taken independent samples t test. The correlations between nonsmall-cell lung cancer MSCT perfusion parameters BV and PMB and expression results of MVD,VEGF,LVD,VEGF-C were analysised respectively.ResultsAll parameters are obtained and the perfusion image is satisfactory.1,There are heterogeneity of VEGF, CD34 expression in NSCLC. The number of positive expression of MVD in the group of positive lymph node metastasis are higher obviously than in the group of non-lymph node metastasis and it have significant difference (P<0.01).2,There are heterogeneity of VEGF-C,D2-40 expression in NSCLC. The number of positive expression of LVD in the group of positive lymph node metastasis are higher obviously than in the group of non-lymph node metastasis and it have significant difference (P<0.01).3,There are positive correlation between perfusion parameter BV and MVD or VEGF of NSCLC and the r value are 0.764 and 0.714 respectively(P<0.01). There are positive correlation between perfusion parameter PMB and MVD or VEGF of NSCLC and the r value are 0.744 and 0.716 respectively(P<0.01). There are no correlation between perfusion parameter BV and LVD or VEGF-C(P>0.05). There are positive correlation between perfusion parameter PMB and LVD or VEGF-C of NSCLC and the r value are 0.727 and 0.667 respectively(P<0.01).ConclusionsIn peripheral non-small cell lung cancer, there are positive correlation between perfusion parameter BV and MVD or VEGF expression, there are positive correlation between perfusion parameter PMB and MVD, LVD, VEGF, VEGF-C expression. High perfusion reflects the biological information that tumor angiogenesis and microcirculation lumen high penetration and low mature, meanwhile it is significance in assessing the aggressiveness and prognosis of tumor.
