The Clinical Research Of 24h Mean Pulse Pressure On Left Ventricular Remodeling In Essential Hypertensive Patients

Objective Essential hypertension(EH) is a one of the diseases threatening health of human being. The research aims at discussing the correlation between 24h mean pulse pressure and target organ's injures in essential hypertensive patients . 24h mean pulse pressure were tested with ambulatory blood pressure determination and hypertensive patients'left ventricular structure and function were measured by Doppler ultrasound technique.Method Essential hypertensive patients were divided into two groups according to 24h mean pulse pressure≥60mmHg, and< 60mmHg by ambulatory blood pressure monitoring. LVDD,IVSD, LVPWD,LVMI and EF were measured by Doppler ultrasound technique in 80 essential hypertensive patients. After adjusting for the confounding effect of MSBP and MDBP by analysis of covariance, the difference of indices of left ventricular remodeling were compared between the two groups. Results The LVDD, IVSD, LVPWD, LVMI are more in the group of 24hPP≥60mmHg than those of 24hPP<60mmHg (P<0.05) . The statistical significance remained after adjusting for the confounding effects of MSBP and MDBP which may affect on left ventricular remodeling (P <0.05). Thus indicated that the increase of PP is a risk factor of left ventricular remodeling.Conclusion The increase of pulse pressure was an important risk factor of left ventricular hypertrophy and left ventricular structure abnormalities. The result indicated that the 24h mean pulse pressure may forecast the left ventricles function injures.Along with the broadening of 24h mean pulse pressure, the odds of left ventricular hypertrophy , systolic function damage and diastolic function damage will increased.Active controling pulse pressure is effective to abate the left ventricles function injures.
