Objective Explore the features of dynamic contrast-enhanced ultrasound sonogram of different pathological types of pelvic mass, Explore the value of time parameters of contrast-enhanced ultrasound in diagnosis and differential diagnosis of benign or malignant of pelvic mass. Methods Collected 110 cases of patients with pelvic mass who came to the gynecological department of the affiliated hospital of Ningxia Medical University for diagnosis and treatment from July 2008 to September 2009, routine ultrasound examinations of patients showed cystic, cystic-solid or solid space-occupying in pelvic mass, all the patients were examined by transvaginal ultrasound, color doppler ultrasound and contrast-enhanced ultrasound at the same time, the results of different methods were compared with the pathological results. The patients were divided into benign and malignant groups by postoperative pathologic results, and then analysed two groups features of dynamic contrast-enhanced ultrasound sonogram. Recorded time parameters of two groups: start enhance time, peak time and wash-out time, and then statistical analysis, P<0.05 for significant difference. Results 1.Contrast-enhanced ultrasound examination to determine the pelvic mass of benign or malignant mass, correct rate is 97.3%,sensitivity is 96.1%,specificity is 98.4%,positive predictive value is 98.0%, negative predictive value is 96.8%, Youden index is 94.5%, positive likelihood ratio is 60.1,significantly higher than the other ultrasonic methods; negative likelihood ratio is 0.04,significantly lower than other ultrasonic methods. 2. Start enhance time, time-to peak of benign group are 19.45±4.76s, 38.00±28.22s, and malignant group are 15.18±5.38s, 28.40±7.94s,benign group is longer than malignant group(P<0.05).Wash-out time of benign group is 188±30.35s,and malignant group is 246±40.86s,benign group is shorter than malignant group (P<0.05). 3. Diagnosis point: benign group start enhance time﹥14s, time to peak﹥30s, Wash-out time<210s; malignant group start enhance time<14s, time to peak<30s, Wash-out time﹥210s. Conclusions Contrast-enhanced ultrasound examination with high correct rate, sensitivity and specificity and other characteristics, by the observation of dynamic contrast-enhanced ultrasound sonogram and the analysis of time parameters, it provides objective basis to diagnosis and differential diagnosis of pelvic mass and helps us detect ovarian cancer earlier, it has wide range of clinical significance and application value.
