[Objective] To explore the value of contrast-enhanced ultrasound in diagnosis of benign and malignant pelvic masses.[Method] The cases from outpatients and in-patients with First Affiliated Hospital of Kunming medical University from October 2014 to January 2016. Among them, the pelvic masses in 62 patients were discovered through transvaginal ultrasound or contrast-enhanced abdominal ultrasound,64 pelvic masses in total. The 64 pelvic masses were examined through contrast-enhanced ultrasound. The followings are observation of pelvic mass contrast agent perfusion processes, analysis of perfusion patter the intensity of perfusion and time-intensity curve, comparison among results of diagnosis and pathology,long-term follow-up.[Results] Among 64 pelvic masses,43 were benign and 21 were malignant. The contrast agent was perfused from surrounding area towards center, equably and equal or low enhancement in benign group. The contrast agent was mostly perfused from center towards surrounding area, rapid no-homogeneity were rapidly enhance, vascular circuit in malignant group. The time-intensity curve appeared mostly gently slow up and gently slow down type in benign group. The time-intensity curve appeared mostly speed up uphill and speed up downhill type in malignant group.Rise time in malignant group (7.70±2.56s)was earlier than in benign group(11.40±6.77s) (P<0.05), peak intensity (PI) (16.30±7.41dB) was more than in benign group(12.12±6.70dB) (P<0.05),area under the curve (AUC) (981.65±548.04dB s) in malignant group was bigger than in benign group(715.22±651.04dB s) (P<0.05),and had statistical significant difference in group, but time to peak (TP),mean transit time (MTT) had no significant difference between the two groups(P>0.05).[Result] Contrast-enhanced ultrasound can show well the blood vessel distribution of mass, analysis of mass contrast agent intensive perfusion patter, time-intensity curve type and parameters help enhancing the ability of diagnosis and distinguishing of benign and malignant pelvic masses.
