Objective: To study the blood flow of Peripheral Lung Cancer with pulse wave Doppler (PW), Power Doppler Imaging (PDI), the blood supply was reflected, hemodynamic Parameters of peripheral pulmonary carcinoma were compared to the expression of vascular endothelial growth factor (VEGF) derived from immunohistochemistry by the characteristics of blood supply and their hemodynamics. The relationship between tumoral vascularity with Doppler sonography and the expression of VEGF in pulmonary carcinoma was investigated, so as to provide a objective evidence for the treatment and assessment of prognosis.Methods: Fifty cases of peripheral lung tumor were studied by 2D-ultrasound, PDI. According to the rich degree of the blood flow with PDI, the pattern of 0~Ⅲwere categorized, 0~Ⅰwas thin blood singnal groups,Ⅱ~Ⅲwas thick blood singnal groups. The pulsatile flows were checked and resistance index (RI) were measured. The value of RI corresponding the highest PSV were recorded. Use Trace function depict total area of blood singnal coverage, namely perfusion area, Formula: perfusion area/bump area of relative perfusion rate. expression of VEGF in tumor was assessed immunohistochemically. Statistical analysis was performed using Chi-square test and Spearman correlation test with SPSS 10.0 software.Results: 50 cases of peripheral lung tumor, 31 patients was detected positive expression of VEGF and the rate is 62.00 percent, which was no relationship with the type of tumor cell (P>0.05). Grade III was significantly higher than that in grade I-II in the expression of VEGF(P<0.05). The positive rates of VEGF rised with diameter, The positive rates of VEGF of two grades is 42.9 percent, 75.9 percent respectively and there were significant differences between-groups. The positive rates of VEGF of stage without metastasis and with metastasis stage were 38.9%, 75.0% respectively. There were significant differences between the positive rates of VEGF of metastasis stage and stage without metastasis. The expression of VEGF showed positive correlation to the relative perfusion rate, blood flow grade(r=0.41, 0.34, respectively, all P<0.05), but showed no significant correlation to RI.Conclusions: 1. The expressions of VEGF positively correlated with the grade of tumor cell, size of tumor, and lymph metastasis of tumor, and it suggested angiogenesis could promote the progression of peripheral pulmonary carcinoma.2. The blood flow grading and relative perfusion rate by CDFI rised with VEGF and can reflect the angiogenesis activity of peripheral pulmonary carcinoma to some extent.3. Doppler sonography of peripheral pulmonary carcinoma can be used as a non-invasive method to evaluate the angiogenesis activity of peripheral pulmonary carcinoma and can Provide a objective evidence of the treatment and assessment of prognosis.
