ObjectiveTo investigate the change of the serum resistin and adiponectin levels after scald and the effect of adiponectin and resistin on the systemic inflammatory response syndrome, and ulinastatin (UTI) as anti-inflammatory drug plays a protective role in visceral organs.Methods①88 Wistar rats were randomly divided into 3 groups. 80 Wistar rats were subjected to 30% TBSA full-thickness scald and divided into scald group (n=40, with intraperitoneal injection of 20ml/kg lactated ringer's solution, bid ) and UTI group (n=-40, with intraperitoneal injection of 5000U/kg UTI in lactated ringer's solution, bid). 8 Wistar rats were as control group. The levels of the serum resistin, adiponectin, TNF-α, IL-6, IL-1β, CRP, MCP-1 and ICAM-1 in rats were measured by ELISA at 3, 6, 12, 24, 48 post-burn hours.②26 patients with severe burns (30%~80% TBSA) were randomly divided into 2 groups: burn group (n=13) and UTI group (n=13, with intraveneous injection of 20000U/kg UTI, tid). 13 healthy volunteers were as control group. The levels of the serum resistin, adiponectin, TNF-α, IL-6, CRP, MCP-1 and ICAM-1 with severe burn patients were measured by ELISA and the levels of the serum IL-1βin rats were detected by EIA in 1, 2, 3, 5, 7, 14 post-burn days. At the same time, the cardiac function, hepatic function and renal function parameters were determined.Result①Compared with control group, the serum level of adiponection was significantly reduced and the serum levels of resistin ,TNF-α, IL-1β, IL-6, ICAM-1, CRP, MCP-1 were obviously increased in scalded group(p<0.05). After treated with UTI, the serum level of adiponection was significantly higher, and the serum levels of resistin, TNF-α,IL-1β,IL-6, ICAM-1, CRP, MCP-1 were obviously lower than those of scalded group(p<0.05). The same changes were observed in severely burned patients.②The level of adiponection was significantly negative correlated with that of TNF-α, ICAM-1, CRP, MCP-1. The level of resistin was significantly positive correlated with that of TNF-α, ICAM-1, CRP, MCP-1.③Compared with burn group, the cardiac function, hepatic function and renal function were improved significantly after treaded with UTI.ConclusionThe resistin , adiponectinthe and inflammatory mediator are involved in the pathogenesis of inflammatory response after burn. Treatment with UTI can markedly regulate resistin and adiponectin secretion, indicating that UTI exerts protective effect on cardiac function, hepatic function and renal function after burn.
