Objective:This study is aimed to investigate the effect of glucocorticoid on IL-27 and its receptor in the myasthenia gravis (MG) patients by detecting the serum concentration of IL-27, the expression of IL-27 receptor subunit WSX-1 mRNA in PBMC, measuring the serum level of anti-AchR-IgG in MG patients before and after glucocorticoid therapy, analyzing the relationship between these results, so as to offer a theoretical basis of the targeted therapy with IL-27.Methods:20 MG patients and 20 healthy controls were investigated by prospective study. The PBMCs was extraced from the peripheral blood. The mRNA expressions of WSX-1 on PBMCs were detected by RT-PCR, the serum concentrations of IL-27 and the levels of anti-AchR-IgG were tested by ELISA, and the correlation among them were analyzed.Results:(1)Glucocorticoid therapy reduced the concentration of IL-27 in serum of MG patient(29.90±3.50 pg/ml VS 27.15±2.45 pg/ml),and the difference were significant (p<0.05). (2) Glucocorticoid therapy reduced the relative OD values of WSX-1 mRNA on PBMCs of MG patients(0.747±0.105 VS 0.567±0.090),and the differences were significant (p<0.05).(3)QMG wasn't positive correlated with the the concentration of IL-27 before or after treatment in the MG group and subset (p>0.05),but the improvement QMG score was positive correlated with reduced concentration of IL-27 after treatment(p<0.05). (4)QMG wasn't correlated with the relative OD values of WSX-1 mRNA on PBMCs of MG group and subset before or after treatment (p>0.05),but the improvement QMG score was positive correlated with the reduction of the latter of MG group andⅡb subset after treatment (p<0.05),however, there was no correlation inⅡa subset (p>0.05).(5) QMG wasn't correlated with the levels of the anti-AchR-IgG before or after treatment, but the improvement QMG score was positive correlated with the latter(p<0.05).(6)The concentration of IL-27 was positive correlated with the levels of the anti-AchR-IgG before and after treatment (p<0.05),similarly, after treatment the reduction of this two index was positive correlated too (p<0.05).(7) The relative OD values of WSX-1 mRNA on PBMCs was positive correlated with the levels of the anti-AchR-IgG before treatment (p<0.05),however, after treatment there was no correlation, but the reduction of this two index was positive correlated (p<0.05).Conclusions:(1)The reduction of the concentration of IL-27 and the anti-AchR-IgG in serum was positive correlated with QMG, indicated that they are expected to be the index of monitoring and assessing the treatment of myasthenia gravis. (2) To reduce the levels of the anti-AchR-IgG by intervention the pathway of IL-27 and WSX-1 in serum might be one of the therapeutic pathogenisis of glucocorticoid treatment for myasthenia gravis.
