ObjectiveThe biomechanical stability of the three different internal fixation methods of a adductive basicervical fracture of femur were explored by the three-dimensional finite element analysis, according to the comparison of the internal fixation methods of the three hollow compression screws with an inverted triangle configuration, a dynamic hip screw system and the configuration of a dynamic hip screw system plus a anti-rotary screw.MethodThree different internal fixation models of the finite element analysis for a adductive basicervical fracture of femur with the above configurations were established for the very investigation with the load of the hip joint pressure and the muscular tensile force, as the situation as standing on one leg.ResultThe overall biomechanical stability of the internal fixation model with the configuration of a dynamic hip screw system plus a anti-rotary screw was better than the other two internal fixation models. The slit of the lateral fracture section fixed by three hollow compression screws with an inverted triangle configuration was the minimum, while the slit of the back frature section was the minimum with the fixation of a dynamic hip screw system. The lateral compact bone stress of the fracture section with the fixation of a dynamic hip screw system was the maximum. The back compact bone stress of the fracture section with the fixation of a dynamic hip screw system plus a anti-rotary screw was the maximum.ConclusionThe three-dimensional finite element analysis of a adductive basicervical fracture of femur was established for the the first time. The three above fixations were available for a simple adductive basicervical fracture of femur, especially the configuration of a dynamic hip screw system plus a anti-rotary screw. The dynamic hip screw system was suitable for the back defect, but not fitting for the lateral defect.
