PurposeTo compare and evaluate the fixing effects of three internal fixation modes forintertrochanteric unstable fractures through three-dimensional finite element method,Compare the advantages and disadvantages of three different internal fixation modes-proximal femoral nail, dynamic hip screw (system) and proximal femoral lockingplate and to provide theoretical mechanics basis for the clinical application.MethodEstablished the ideal finite element models of two kinds of intertrochantericunstable fractures31-A2and31-A3as well as the finite element models of threedifferent internal fixation methods, to simulated a situation of human’s normalwalking gait (one-foot neutral position while walking), and loaded the models with ajoint pressure of an assumed overall weight0f60KG, and respectively carried out thefinite element calculations of these models. Analyzing the proximal femoraldisplacement, internal fixations displacement, fixing components stress, Fracturecross-section stress, calcar femorale stress and Proximal femoral Cancellous bonestress, compare the advantages and disadvantages of three different internal fixationmodes.ResultsIn the fractures of31-A3type the three kinds of internal fixation methods may beused, but PFN showed the best mechanical stability, and the mechanical stability ofproximal femoral locking plate is the second best, and the mechanical stability ofDHS is at the third place. and DHS was not guited to the fractures of31-A3typeConclusionTo compare the biomechanical properties of three kinds of internal fixationapplications in unstable intertrochanteric fractures-proximal femoral nail (PFN),dynamic hip screw (DHS), proximal femoral locking plate (PFLCP) by usingthree-dimensional finite element method,This method is feasible.In the fractures of 31-A2type, the above three kinds of internal fixation methods may be used, but PFNshows the best mechanical stability, and the mechanical stability of proximal femorallocking plate is the second best, and the mechanical stability of DHS is at the thirdplace. In the fractures of31-A2type, PFN shows the best mechanical stability,followed by the mechanical stability of proximal femoral locking plate, and DHS doesnot apply to this type of fractures.
