Objective Bladder transitional cell carcinoma(BTCC) is one of the most common tumor to the urinary system,it easy to recur because of high degree of tumor malignant,we cannot reach good therapeutic efficacy.Transferrin receptor(TfR) is a kind of transmembrance glycoprotein in cellular membrance,it mediate absorption of iron through interreaction with transferrin,has close relation with grouth, proliferation, differentiation and canceration of cell.We observe the expression of TfR in different grades and stages BTCC and normal bladder tissue,and analyze the relation of expression with pathology and different pathological classification,provide a scientific basis for the occurrence of bladder cancer, development, proliferation and differentiation of basic research and study as a prognosis of bladder cancer.Method We collect 40 samples of bladder cancer and 10 cases of normal bladder mucosa from October 2006 to February 2008 in department of urology of XiangYa hospital of Central South University and Affiliated Hospital of Hainan Medical College,32 cases of bladder cancer were Performed partial cystectomy, full-line of resection in 8 cases. Examine the expression of TfR in BTCC and normal bladder tissue by immunohistochemistry and analyze the relationship of TfR expression of BTCC in different clinical and pathologic grades and normal tissue.In addition,we do periodic postoperative follow-up to all cancer patients,compare the the expression of TfR in pre-recurrence and post-recurrence stages after surgery, provide the scientific basis for research of prognosis.Result The result show that the expression of TfR in both different grades BTCC and normal bladder is positive according to immunohistochemistry of histopathologic slide.The point of TfR expression in the normal bladder mucosa is 2.8±1.2, BTCC is 7.5±1.6,the point of BTCC is two times higher than normal tissue(P<0.01),the point of TfR in Tis stage of 8 cases of BTCC is 5.8±1.1,T1 stage of 10 cases is 6.6±1.2,T2 stage of 8 cases is 7.1±1.4, T3 stage of 7 cases is 7.8±1.2,T4 stage of 7 cases is 8.5±1.3,the expression of TfR in high-grade BTCC is higher than low-grade(P<0.01),there is positive correlation between the two;The point of TfR expression in G1 stage of 12 cases BTCCis 6.2±1.7,7.0±1.6 in G2 stage of 13 cases,8.1±1.8 in G3 stage of 15 cases,there is significant differences between level of the expression of TfR and pathological grades and differentiation grades,the higher of pathological and the lower of differentiation,the higher of TfR.Conclusion①The expression of TfR in BTCC is higher than normal bladder tissue;②The expression of high-grade BTCC is higher than low-grade;the higher of pathological and the lower of differentiation, the higher expression of TfR;③TfR play a part in physiological and pathological processe of cells;④There are higher expression of TfR in recurrence of BTCC, TfR can provide a scientific basis for the prognosis to BTCC and can be used as an early prognostic factor.
