| Objective:Analysis and comparison of the therapeutic effect of t he treatment of intertrochanteric fractures in elderly patients with He miarthroplasty and DHS Internal Fixation, to approach the clinical sig nificance of the hemiarthroplasty for elderly patients with intertrochan teric fractures.Methods:Retrospective analysis 11 case of the intertrochanteric fractures in elderly patients who were treated by hemiarthroplasty in our hospital between March 2008 and January 2010.4 cases of them are male,7 cases of them are femal, all the patients are at an averageof 85-year-old(ranged 78-99 years). Select the intertrochanteric fracture cases who were treated by DHS internal fixation and had the complete information for comparison. A total of 15 cases,5 cases of them are male,10 cases of them are femal, all the patients are at an average of 73.3-year-old(ranged 64-93 years).2 groups are injured when they fall injuries. According to Tronzo-Evens, Hemiarthroplasty group:8 cases of typeâ…¢,3 cases of type IV; DHS group:11 cases of typeâ…¡,4 casesof typeâ…¢. Recorded the time and blood-transfusion during operation in both groups, and fluped the ambulatory time, the occurrence of complications and Harris scores after operation.Results:There was no difference in operation time between the hemiarthroplasty group and the DHS internal fixation group(P>0.05); The time to ambulation in the hemiarthroplasty group was shorter than that of the DHS internal fixation group(P<0.01); The blood-transfusionduring operation was compared in the two groups by Rank sum test and it showed that there was no difference in the volume of blood-transfusion between the hemiarthroplasty group and the DHS internal fixation group(P>0.05); There was no significant difference in postoperative complications incidence rate between the two groups, but there were no complications related to lying in bed and caging in the hemiarthroplasty group, while there were many postoperative complications in the internal fixation group because of the long time lying in bed; TheHarris scores after operation was compared in the two groups by X2-test and it showed that the curative effect of the hemiarthroplasty groupwas better than the DHS internal fixation group (P<0.05).Conclusions:Hemiarthroplasty can be one of the therapies to the intertrochanteric fractures in the eldly, which has a value in short-term therapeutic effects, which can allow patient to get out bed earlier for weight-bearing, and to recover with pre-injury activities earlier, which can reduce complication and improve the quality of life inpatients. |
