The Clinical Value Of The Serum Heat Shock Protein 70 In The Diagnosis Of Myocardial Ischemia-reperfusion Injury

Objective:To investigate the clinical value of serum heat shock protein 70 (HSP70) in myocardial ischemia-reperfusion injury of the cardiopulmonary bypass(CPB) patients. Methods:We divied 30 patients with open-heart surgery in CPB into two groups :15 of them with aortic cross- clamping operation group(control group) and 15 of them without aortic cross-clamping operation group(experimental group).We draw the patient's blood before operation and after reperfusion 1 hour ,3 hours ,5 hours and 12 hours respectively.Then detected cardiac-specific troponin T(cTnT) in automatic biochemical analyzer and HSP70,glycogen phosphorylase BB(GP-BB)with enzyme-linked immunosorbent assay(ELISA). Results: The experimental group vs control group: serum concentration of HSP70 was significantly higher [preoperation 30 minutes: (309.7±38.70) vs. ( 309.9±38.42),t=-0.105,p=0.917;1h:(556.4±27.79)vs.(386.4±36.40), t=-14.376,p=0.000;3h:(666.1±40.49)vs.(402.9±36.19),t=-18.767,p=0.000;5h:(594.9±30.82)vs.(381.1±28.37),t=-19.764,p=0.000,12h:(529.3±63.43)vs.(347.7±50.82),t=-8.652,p=0.000], serum concen- trations of cTnT and GP-BB was significantly higher. In the experimental group the serum concentration of HSP70 was significantly higher in post-operation than in pro-operation[1h:(556.4±27.79)vs.(309.9±38.42),t=-20.131,p=0.000;3h:(666.1±40.49)vs.(309.9±38.42),t=-24.716,p=0.000;5h:(594.9±30.82)vs.(309.9±38.42),t=-22.410,p=0.000,12h:(529.3±63.43)vs.(309.9±38.42),t=-11.460,p=0.000], serum concentrations of cTnT and GP-BB was significantly higher in post-operation than in pro-operation. The serum concentrations of HSP70 and cTnT, GP-BB concentration were significantly positively correlated.. Conclusion: Serum HSP70, cTnT, and GP-BB were higher in patients with aortic cross-clamping of open heart surgery than in without aortic cross-clamping. There were positive correlations between the serum concentration of HSP70 and the cTnT , GP-BB ,both of which are known myocardial ischemia's indicators.We concluded that serum HSP70 has a certain significance in dignosis of myocardial ischemia-reperfusion injury in CPB patients.
