The Relationship Between IGF-1 Levels And Inflammatory Factors, Traditional Chinese Medicine Syndrome Type In Patients With Type 2 Diabetes Mellitus

Objective:To study the relationship of IGF-1 levels and inflammatory factors related to type 2 diabetes, and their distribution in the different types of TCM Syndromes. It may provides a reference for the Western study of the mechanism of type 2 diabetes, and the objective of TCM syndromes, but also may provide a useful exploration on the pathogenesis of type 2 diabetes.Methods:We Collected 65 cases of patients firstly diagnosed as type 2 diabetes and 10 cases of normal control group of 65 cases,16 cases were dry-heat with yin deficiency syndrome,18 cases were deficiency of both qi and yin syndrome,16 cases were retention of phlegmatic dampness syndrome,15 cases were blood stasis of collateral syndrome. The patients were examined the targets as below:Insulin-like growth factor-1 (IGF-1), hs-CRP, IL-6, body mass index(BMI), waist circumference, fasting plasma glucose(FPG), HbAlc, lipid-related indicators (TC, TG, HDL-C, LDL-C).Results:1.Using correlation analysis, in type 2 diabetic patients, the serum IGF-1 was negatively correlated with hs-CRP and IL-6 (r=-0.345,P=0.005<0.01; r=-0.211,P=0.014<0.05), and was positively correlated with BMI and FPG (P=0.008<0.01; P=0.016<0.05). Serum hs-CRP was positively correlated with IL-6 (P<0.01).2.Using stepwise regression analysis, in type 2 diabetes, taking IGF-1 as dependent variable, hs-CRP and BMI were significantly affected on IGF-1 (P<0.01; P<0.01);taking hs-CRP as dependent variable, IL-6 were significantly affected on hs-CRP (P<0.01);taking IL-6 as dependent variable, hs-CRP were significantly affected on the IL-6 (P<0.01).3.Compared with the groups of type 2 diabetes TCM syndromes, the IGF-1 levels of normal control group were higher, the IGF-1 of the group of retention of phlegmatic dampness syndrome is the highest than other groups of type 2 diabetes TCM syndromes, the difference was statistically significant (P<0.05), but the difference is not disappear in the group of dry-heat with yin deficiency syndrome,the group of deficiency of both qi and yin syndrome and the group of blood stasis of collateral syndrome; Compared with the groups of type 2 diabetes TCM syndromes, the hs-CRP and IL-6 levels of the normal control group were lower, the hs-CRP and IL-6 levels of the group of retention of phlegmatic dampness syndrome and blood stasis of collateral syndrome were higher than other TCM syndromes, the difference was statistically significant (P<0.05). Additionally, compared with the group of dry-heat with yin deficiency syndrome, deficiency of both qi and yin syndrome and blood stasis of collateral syndrome,the body mass index (BMI), fasting plasma glucose (FPG) of the group of retention of phlegmatic dampness syndrome were the highest, the difference was statistically significant (P<0.05). Compared with the group of dry-heat with yin deficiency syndrome, deficiency of both qi and yin syndrome,the low-density lipoprotein (LDL) of the group of retention of phlegmatic dampness syndrome and the group of blood stasis of collateral syndrome were both higher, the difference was statistically significant (P<0.05).Conclusions:1.Compared with the normal, the serum IGF-1 levels of Type 2 diabetes was decreased, the levels of hs-CRP and IL-6 levels increased, and IGF-1 levels and hs-CRP, IL-6 levels show a significant negative correlation. It suggested that the decrease of IGF-1 levels may be related with the low-grade inflammation state of type 2 diabetes.2. The CRP and IL-6 levels of the group of retention of phlegmatic dampness syndrome and blood stasis of collateral syndrome were higher than other groups', suggesting that the inflammatory states of type 2 diabetes may be the one of the pathological essential of the retention of phlegmatic dampness syndrome and blood stasis of collateral syndrome.3. The IGF-1 of the group of retention of phlegmatic dampness syndrome is the highest than other groups of type 2 diabetes TCM syndromes, but the difference is not disappear in the group of dry-heat with yin deficiency syndrome,the group of deficiency of both qi and yin syndrome and the group of blood stasis of collateral syndrome.
