SHENG JI YU HONG Plasters has the effects of promoting blood flow, detoxification, removal of rot, promoting granulation, and the clinical used for wound which ichor to exhaust and slow growth of granulation. Gelatin sponge, as a hemostatic implantation of drug has the feature of safty, non-toxic and is used in clinical long-term. Gelatin is a collagen product fractured by a moderate, it has the same chemical composition of collagen, retained most of the excellent physical and chemical properties, which features the promotion of cell adhesion and growth performance, and has good tissue affinity, while with activated macrophages and hemostasis in the wound is bleeding, decreased wound exudate, to avoid the wounds dry and antibacterial effect.The lesson is on the basis of the Prof.Yao Chang's early experiences in SHENG JI YU HONG Ointment and angiogenic materials, with the modification of collagen, producing SHENG JI YU HONG herbs loading on modified gelatin sponges carried out. The effect of promoing the growth of granulation and treating in chronic leg ulcers will be preliminary observed, related mechanism will also be discussed in the article.First:Experimental studyObjective:To evaluate the effects of SHENG JI YU HONG herbs loading on modified gelatin sponges on the promoing the growth of granulation and explore related mechanism.Mthods:Full thickness back skin was cut mechanically and covered withn petrolatum gauzes, modified gelatin sponges, SHENG JI YU HONG plasters and SHENG JI YU HONG herbs loading on modified gelatin sponges respectively. At day3, day7 and day 14 after model made, the wound surface healing rate, the amount of hydroxyproline, hemoglobin, the number of capillaries, cells with positive bFGF and the contents of collagenâ… ,â…¢by immunochemical method inside granulation of the wound were detected individually.Results:At day3, the amount of hydroxyproline, hemoglobin, the number of capillaries, cells with positive bFGF and the contents of collagen I of SHENG JI YU HONG herbs loading on modified gelatin sponges were significant enhanced than the other groups. At day7, all the experimental indices of SHENG JI YU HONG herbs loading on modified gelatin sponges were better than the other groups with P<0.05 and P<0.01,except the contents of collagenâ…¢. At day 14, the contents of collagen I of SHENG JI YU HONG herbs loading on modified gelatin sponges was no significant difference than the other groups, while cells with positive bFGF had significant difference.Conclusion:One of the mechanisms of improving wound and reducing scar of SHENG JI YU HONG herbs loading on modified gelatin sponges is to adjust the content of bFGF, collagenâ… ,â…¢inside granulation of the wound and thus promoting angiogenesis with better microcirculations there at the process of would healing. The effects on wound healing of SHENG JI YU HONG herbs loading on modified gelatin sponges is better than those of traditional SHENG JI YU HONG plasters.Second:Clinical observationObjective:To evaluate the effects of SHENG JI YU HONG herbs loading on modified gelatin sponges on the clinical efficacy of chronic leg ulcers.Mthods:The sixty cases of the chronic leg ulcers patients:were enrolled in study and randomly divided into two groups, treatment group(thirty-two cases as SHENG JI YU HONG herbs loading on modified gelatin sponges) and contol group(twenty-eight cases as SHENG JI YU HONG Plasters).Every goup was treated by changing dressing next day, and the observation lasted eight weeks. The wound area and the granulation color were detected individually once a week.Results:At week 2 and 4, the wound surface healing rate of treatment group was significant enhanced than the control groups. At week 2 and 3, the granulation color of treatment group was better than the control groups. Eight weeks later, the time of healed patients in treatment group was shorter than the control group, and the markedly effective rate was significant higher than the control group with P<0.05 and P<0.01.Conclusion:SHENG JI YU HONG herbs loading on modified gelatin sponges is to improve wound microcirculations and thus eliminating necrosis, accelerating the growth of granulation and wound healing. The clinical effect on wound healing of SHENG JI YU HONG herbs loading on modified gelatin sponges is better than those of traditional SHENG JI YU HONG plasters.
